What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

DateWhatDetail
May 21, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) cannot legally disclose if a complaint was also received or investigated by any other agency. OCCL investigations of regulatory non-compliance begin when clearance from other agencies, when applicable, has been received. On April 25, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to follow policies regarding the release of children. After interviewing staff, the Reporter, the owner, and the administrator and reviewing center policies, child files, and attendance records, there is no evidence to substantiate this complaint for not following policies of releasing children only to people approved by a parent or guardian. All center staff interviewed stated there have been no recent instances of a child being released to the wrong person. A review of child files documented an injury report signed and dated by Parent #1 on April 24, 2025, indicated Parent #1 picked up Child #1 on that date, not Person #1.
Apr 1, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing received a complaint on March 10, 2025, regarding allegations of a lack of interactions with infants, child file requirements, and lack of supervision. An investigation was conducted on March 13, 2025. Based on interviews with the administrator and staff, and a review of medical records, there is no evidence to substantiate this complaint for a lack of interactions with infants, child file requirements, or a lack of supervision.
Nov 13, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn September 5, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to provide an incident report within one business day and failing to notify parent or pick-up person of an injury before the child leaves for the day. An investigation was begun on September 13, 2024, and concluded on October 28, 2024. There is no evidence to substantiate this complaint for failing to provide an incident report within one business day, as an incident report was provided and signed dated September 5, 2024. There is no evidence to substantiate this complaint for failure to notify the family of a child injury because, although two staff stated in verbal interviews that they did not know of the bite until informed by Person #1, there is a signed injury report dated the date of the injury with first aid provided to the child. Staff #1 stated Person #1 was informed at pickup on September 4, 2024, there had been an injury to Child #1 and there was a report to be signed at the front desk. It is noted that the center copy and family copy of the accident report have different dates it was signed. Also noted is Administrator #1 s statement staff were unaware of the bite mark until Person #1 notified them on September 4, 2024, but Staff #1 wrote in the injury report that Staff #1 provided basic first aid to Child #1 and recorded this in the accident report. There is apparently confusion among center staff regarding this incident, but all statements confirm the family was notified according to regulatory requirements.
Jun 7, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn May 14, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding not making reasonable accommodations for a child with a disability and submitting false claims to Purchase of Care. An investigation was conducted on May 17, 2024. There is no evidence to substantiate this complaint for not making reasonable accommodations for a child with a disability based on interviews conducted. The staff interviewed stated they wanted to get clarification on requirements for the therapist and never stated Child #1 could not attend. There is no evidence to substantiate this complaint for false claims to Purchase of Care based on Purchase of Care stating there were no claims submitted for Child #1 for the month of May.
Dec 1, 2023OCCL Standards Complaint: No Evidence to SubstantiateOn November 7, 2023, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of improper discipline, not notifying parents of an injury, not documenting the injury, and not having water available to the children. An unannounced visit was conducted on November 14, 2023, to investigate this complaint. There is no evidence to substantiate this complaint because the administrator and staff members interviewed stated there had not been any accidents or injuries in November in the two-year-old classroom. The staff members also stated they do not handle the children roughly and they have not seen other staff members handle the children roughly. Ms. Jackson stated when an accident or injury does occur, the parent is called and messaged through the ProCare application. It was observed during the complaint investigation that neither of the staff members in the two-year-old classroom had long fingernails that could have caused scratch marks. Pitchers of water were observed in the center. The kitchen provides a pitcher of water to each classroom for the children to drink throughout the day.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
