The short answer

Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.

Each figure’s source sits beside it, in its section below.

What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

What did inspectors find?

Delaware’s list shows only the visits that found violations, so a zero can’t be read either way: this provider may have had visits that found none, or no visit at all. No count is shown.

Sep 16, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn August 27, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to provide an incident report. An investigation began on September 2, 2026, and concluded on September 3, 2026. Based on staff interviews and documents reviewed, there is no evidence to substantiate this complaint for failing to provide an incident report. All staff members interviewed stated they did not view Child #1 cry or seem in distress during the day on August 26, 2026. Staff also stated they were not aware of any scratches on Child #1 as they were covered by their shirt.
Sep 14, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn August 19, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to use appropriate behavior supports. An investigation began on August 25, 2026, and concluded on August 26, 2026. Based on staff interviews, and video reviewed, there is no evidence to substantiate this complaint for failing to use appropriate behavior supports. Administrator #1 stated Child #1 was kicking and screaming and did not calm down when placed in the library area. Administrator #1 stated they were concerned Child #1 would hurt themself. The video footage showed Administrator #1 lifting Child #1 off the floor into a child-sized chair, holding the chair in position so Child #1 cannot get up, turning the chair toward Parent #1 who had just arrived, then within a few seconds allowing Child #1 to get up and go to Parent #1. DELACARE regulations state, If the child or others are in imminent danger or risk to themselves or others, physical holding for as brief as possible to remedy the risk or removing the child to a safer location is permissible.
Sep 24, 2024OCCL Standards Complaint: SubstantiatedOn September 4, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding the center s food service policy for addressing food allergies, serving food to a child with known allergies, and not immediately providing medical attention, then notifying parent when child has an allergic reaction. An investigation was conducted on September 9, 2024. Based on observations, interviews with staff, and documentation, this complaint is substantiated for not following the center s food service policy addressing food allergies. Although Staff #1, Staff #2, and Administrator #2 noticed Child #1 s menu was not highlighted indicating it was permissible to feed them chicken salad, they still served Child #1 the chicken salad sandwich. Based on observations, interviews with staff, and documentation reviewed, this complaint is substantiated for serving food to a child with known allergies. Although Staff #1 and Staff #2 noticed Child #1 was not permitted to have the chicken salad sandwich, they prepared and served the sandwich to them. Based on interviews with the staff there is no evidence to substantiate for not immediately providing medical attention before notifying the parent. Staff #1 and #2 both stated Child #1 had no reaction after eating the sandwich, or before nap. Staff #1 stated Child #1 only vomited and started coughing towards the end of naptime, a few hours later. At that time, the action plan was followed, and the parent was notified. An additional concern noted after a review of documentation was that the Medication Administration Record (MAR) form for Child #1 had not been documented as having been reviewed and updated yearly as required.
May 3, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn April 19, 2024, the Office of Child Care Licensing received a complaint regarding inappropriate communication by staff with a child and policies for handling parent complaints. An investigation was begun on April 26, 2024, and concluded on April 30, 2024. Based on the results of this investigation, there is no evidence to substantiate this complaint because the center administrator, Staff #1, and Staff #2 all stated they have never observed staff speaking to children inappropriately. Also, the center administrator stated she did speak with Child #1 s parent on the phone regarding her concerns and offered her times to meet in person.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
