What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Nov 12, 2025OCCL Standards Complaint: SubstantiatedOn October 17, 2025, the Office of Child Care Licensing (OCCL) received a complaint for allegations of staff not being respectful while speaking or interacting with others and staff not respecting children and their needs. After interviews with staff members and administration, this complaint is substantiated for Staff #1 failing to be respectful when speaking to children at the center.
Mar 4, 2025OCCL Standards Complaint: No Evidence to Substantiate with ConcernThe Office of Child Care Licensing received a complaint on February 10, 2025, regarding allegations of incorrect staff-to-child ratios. On February 18, 2025, additional concerns regarding termination policies, transition policies, and use of cell phones by staff while supervising children were received by OCCL from the same Reporter. An investigation began on February 13, 2025, and concluded on February 21, 2025. There is no evidence to substantiate this complaint for incorrect staff-to-child ratio because observations and attendance records for two classrooms showed all classrooms were in correct staff-to-child ratio. There is no evidence to substantiate this complaint for failing to follow termination policies or transition policies because the administrator stated no child had been terminated from care and Parent #1 signed and agreed with the transition policy. There is no evidence to substantiate this complaint for staff using cell phones while caring for children because the administrator stated staff only use cell phones to communicate with parents during planning time, not while supervising children. An additional concern noted at the visit was LS #1 observed Staff #1, who had not had a criminal background check completed was working with children. LS #1 notified Administrator #2 that Staff #1 could not be present until they were fingerprinted and determined at least provisionally eligible. The staff member left the building.
Mar 20, 2024OCCL Standards Complaint: Unsubstantiated with ConcernOn February 19, 2024, the Office of Child Care Licensing received a complaint regarding concerns about the indoor temperature being under the required minimum temperature of 68 degrees. This complaint is unsubstantiated with concern. When checked by an OCCL thermometer used for verifying room temperatures, the temperature in the infant and one-year-old classrooms registered 68 degrees. The two two-year-old classrooms thermometers read 63 degrees, but the Licensing Specialist s thermometer registered 68 degrees. The three and four-year-old classrooms thermometers read 68 degrees, and 69 degrees on the Licensing Specialist s thermometer. The five-year-old classroom s thermometer and the Licensing Specialist s thermometer read 68 degrees. The classroom temperatures were within the required temperature range during both the complaint and follow-up visits. Additionally, the temperature is not locked as Staff #5 was able to change the temperature in her classroom. A concern noted at this visit was that Staff #1 left her four infants alone in the classroom, while they were asleep, to assist the Licensing Specialist with the classroom walkthrough. Staff #1 accompanied LS Hackett during her entire visit. An additional concern was that the infants were observed asleep with blankets and bottles in their cribs.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
