The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Aug 12, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On August 4, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of failure to use positive behavior supports and not using respectful communication with families. An investigation was conducted on August 10, 2026. There is no evidence to substantiate this complaint for failing to use positive behavior supports based on interviews conducted and observations of staff. Administrator #1 and three staff members stated they only observed and used positive behavior supports within the facility. All staff members interviewed stated they use positive behavior supports. There is no evidence to substantiate this complaint for not using respectful communication with families based on staff interviews. Administrator #1 and three staff members interviewed stated they have only observed respectful communication within the facility. All staff members interviewed denied using disrespectful communication. Interactions observed during the visit were positive. |
| Jun 9, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On May 14, 2026, Licensing Specialist (LS) #1 was notified of a child injury by Administrator #1 during an unannounced compliance review. At the time of this notification, LS #1 received relevant documentation regarding this injury. On May 15, 2026, Office of Child Care Licensing (OCCL) received a complaint regarding allegations of inadequate playground supervision resulting in an injury to Child #1 during outdoor play activities. An investigation was conducted on May 26, 2026. Based on the investigation, including interviews with Administrator #1 and review of documentation, there is no evidence to substantiate this complaint for inadequate supervision. Administrator #1 and Staff #3 both stated they observed multiple staff members positioned together on a bench area in the shade while children engaged in playground activities. However, Administrator #1 stated the staff were seen on the video footage visually monitoring the children, looking around at different areas of the playground but stated they were not actively moving around. There is no evidence to substantiate the complaint for a failure to create an injury report because Administrator #1 stated Staff #2 contacted the family immediately via telephone, and Staff #3 confirmed a call was made and an injury report was produced but declined by Parent #1. LS #1 received a copy of this injury report. |
| Jan 2, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On December 11, 2025, the Office of Child Care Licensing (OCCL) received a complaint concerning the administrative team failing to follow proper exclusionary procedures related to the identification of head lice within the program. An investigation was conducted on December 17, 2025. Based on the investigation, there is no evidence to substantiate this complaint for the program failing to follow exclusionary procedures related to head lice. Documentation obtained from the Brightwheel application indicated all three identified children were excluded from care for a minimum of 24 hours after beginning treatment for head lice. |
| Nov 12, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On October 29, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding a failure to notify parents of an injury as well as a lack of supervision. An investigation was conducted on November 5, 2025. Based on the investigation, there is no evidence to substantiate the complaint of failure to notify the parents. The investigation determined the program was unaware of any injury when Child #1 was picked up from the program on October 23, 2025. Administrator #1 did not receive notice of Child #1 being evaluated at the hospital for an elbow injury until October 27, 2025, through the Brightwheel communication application. Documentation reviewed by LS #1 confirmed communication between Administrator #1 and Parent #1 regarding the injury, and a detailed follow-up report was provided to the family upon completion of the program s internal investigation. There is also no evidence to substantiate the complaint for a lack of supervision. During the investigation, LS #1 toured the classroom and confirmed all areas of the room were visible from multiple vantage points. Staff #1 was interviewed and stated they did not observe Child #1 fall or injure themselves at any time. LS #1 reviewed video footage from October 23, 2025, and also did not see any instance in which Child #1 was injured. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
