What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Jul 27, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernThe Office of Child Care Licensing (OCCL) received a complaint on June 22, 2026, regarding concerns of providing transportation without written parent permission. An investigation was conducted on June 29, 2026, which included interviews with Administrator #1 and Staff #1 and a review of attendance records, transportation procedures, and parent communications. There is no evidence to substantiate this complaint regarding providing transportation without written parent permission or not following the facility s transportation procedures. Information obtained during the investigation showed Parent #1 informed staff at drop-off Child #1 would be transported by a contracted bus to a summer program, and staff followed Parent #1’s direction by placing Child #1 on the bus. Current DELACARE Regulations only require written transportation arrangements when the facility is the one providing transportation. A concern noted during this investigation was Child #1 was not signed out on the attendance record when departing on the bus.
Jul 22, 2025OCCL Standards Complaint: No Evidence to Substantiate with ConcernThe Office of Child Care Licensing received a complaint on April 16, 2025, regarding lack of supervision and positive behavioral supports. An investigation was conducted on April 30, 2025, which included observation of transition procedures and interviews with staff members. There is no evidence to substantiate the complaint regarding a lack of positive behavior supports. The discipline practices observed while the children transitioned from school and also when they were on site were positive and appropriate, and the children were in a clear routine. There is no evidence to substantiate this complaint for inadequate supervision. Due to the high volume of children, parents, and families crossing the street simultaneously, staff had no reasonable way to identify Child #1 was walking with the group until reaching the front entrance, as multiple other children were observed using this parking lot. It was also noted many parents wait in the parking lot to pick up their children, in an effort to avoid the school s pick-up line. A concern noted during this investigation was the staff member picking children up from the school-age program did not have attendance records or a checklist to notate all children that were being picked up from the elementary school and walked to the facility.
Jul 10, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn May 23, 2024, a complaint was called into the Child Abuse and Neglect Report Line and accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 23, 2024, and accepted it for investigation regarding improper discipline. An investigation began on May 23, 2024, and was concluded on May 30, 2024. There is no evidence to substantiate this complaint for improper discipline based on interviews of two staff members and Owner Lola Mayo. Staff #1 and Ms. Mayo both stated they did not observe Staff #2 grab Child #1 on the arm. Staff #2 denied grabbing Child #1, and stated she only verbally requested Child #1 move to another seat and be quiet while someone was on the telephone.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
