The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Aug 12, 2026 | OCCL Standards Complaint: Substantiated | On July 21, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to follow diapering regulations, failing to have and follow procedures to address food allergies, child file requirements, and failing to follow menu requirements. An investigation was conducted on July 28, 2026. Based on interviews and observations made during the investigation, this complaint is substantiated for failing to follow diapering regulations. On live video, LS #1 observed Staff #1 changing a child’s diaper on a mat with a disposable cover on the classroom floor rather than using the designated changing table in the adjoining classroom. Staff #1 disinfected the mat as required after use. It was also observed that Staff #1 did not wash their own hands or the child’s hands following the diaper change. Administrator #1 confirmed staff is expected to use the adjoining classroom for diaper changes. This complaint is also substantiated for failing to follow menu requirements. The food served for breakfast the day of the visit did not match the posted menu for that day. There is no evidence to substantiate this complaint for allegations regarding children with allergies not receiving appropriate substitutions, failing to have and follow procedures to address food allergies, and child file requirements. Administrator #1, Cook #1, and staff consistently stated children with dietary needs and allergies receive appropriate substitutions. LS #1 verified allergy documentation in child files and observed allergy lists posted in the office and classrooms. |
| Jul 29, 2026 | OCCL Standards Complaint: Substantiated | On June 26, 2026, the Office of Child Care Licensing (OCCL) received three complaints from the same reporter regarding allegations of administration of medication requirements, lack of supervision, not having a designated, qualified person in charge in the absence of the administrator, incomplete child file information, transportation regulations, positive behavior supports, disrespectful communication by staff, and failing to meeting the needs of children. An investigation began on June 29, 2026, and concluded on July 6, 2026. Based on interviews with the director and staff, and a review of child files, this complaint is substantiated for incomplete child files and for not following the requirements for administering medication to children because Child #1’s file did not contain a completed Medication Administration Record (MAR) for the medication that three staff members who were interviewed stated had been administered to Child #1 on June 17, 2026. Child #1 was administered duplicate doses of an over-the-counter medication without an accompanying MAR and not in its original packaging. This complaint is substantiated for lack of supervision because Licensing Specialist #1 observed video footage from multiple dates, showing a child alone in a hallway without a staff member supervising. This complaint is substantiated for staff not meeting the needs of children because all staff interviewed stated they had observed staff members picking up and moving children on cots while they were sleeping. There is no evidence to substantiate this complaint for failing to have a qualified person in charge in the absence of the administrator because the center notified OCCL of the former administrator s resignation within the required timeframe and they are currently searching for a permanent replacement. Based on interviews and a review of documentation there is no evidence to substantiate this complaint for failing to meet transportation regulations, failing to use positive beh avior supports, or disrespectful communication by staff. |
| Apr 21, 2026 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | On March 26, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of incorrect staff-to-child ratio, staff failing to follow diapering and soiled clothing procedures, using inappropriate behavior supports, and failure to report suspected abuse or neglect of a child. An investigation began on March 30, 2026, and concluded on April 13, 2026. After interviews with the administrator, interviews with staff, and a review of attendance records, there is no evidence to substantiate this complaint for incorrect staff-to-child ratio, staff failing to follow diapering and soiled clothing procedures, using inappropriate behavior supports, or failure to report suspected abuse or neglect of a child. Although one staff member stated they heard another staff member telling a child they could earn their food back tomorrow, the administrator and five other staff all stated they had no knowledge of this occurring. An additional concern noted was staff attendance record documentation was incorrect. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
