What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Feb 11, 2026IA Investigation: Unsubstantiated with ConcernDelaware gives no summary.
Jan 27, 2026OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint on January 9, 2026, regarding concerns of lack of supervision due to staff being inattentive to their classrooms, failure to use positive behavioral supports within the classrooms, and blocking exits in classrooms. An investigation was conducted on January 13, 2026. This complaint is substantiated for blocking exits in classrooms based on interviews conducted. While no blocked exit was observed at the time of the visit, Administrator #1 and two staff members stated an exit of a classroom was covered by furniture to prevent a child from running out of the facility. There is no evidence to substantiate this complaint for lack of supervision due to staff being inattentive to their classrooms, based on interviews conducted and observations at the time of the visit. Administrator #1 and six staff members interviewed all stated they are attentive to children at all times. There is no evidence to substantiate this complaint for concerns of not following positive behavior supports, based on staff interviews conducted. Administrator #1 and six staff members all stated they have only observed appropriate positive behavior management within the facility and stated they only use appropriate behavior management support. Additional concerns were noted. In the review of attendance records from January 5, 2026, to January 9, 2026, staff members who are not qualified through DEEDS Early Learning were working alone with children on multiple occasions. A staff member was documented as working with children on multiple occasions without having received a comprehensive background check eligibility or provisional eligibility determination. In addition, attendance records were inaccurate, as there were several instances of staff not signing out of their classrooms from January 5, 2026, through January 9, 2026.
Jan 14, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernThe Office of Child Care Licensing (OCCL) received a complaint on January 2, 2026, regarding concerns of not using appropriate behavior management, lack of supervision due to children being bitten, not following diapering procedures, and not following meal/snack requirements by allowing children to eat off the floor. An investigation was conducted on January 2, 2026. There is no evidence to substantiate this complaint for lack of supervision, leading to excessive biting in classrooms based on interviews with eight staff members and observation of the classrooms. All staff interviewed stated they have not had any issues with recurring biting occurring in the classrooms. There is no evidence to substantiate this complaint for concerns of not following positive behavior supports, based on staff interviews conducted. Eight staff members were interviewed separately and stated they have only observed appropriate positive behavior management within the facility and stated they only use appropriate behavior management support. There is no evidence to substantiate this complaint for concerns of not following diapering procedures based on staff interviews and observations of diaper changes. All staff members interviewed stated diapers are checked every hour, changed every two hours, or changed immediately if soiled. Observations of diaper changes show staff members following required diaper-changing procedures. There is no evidence to substantiate this complaint for concerns of not following meal/snack requirements by allowing children to eat off the floor based on staff interviews conducted. All staff members interviewed stated children do not eat off the floor. Two staff members stated when children go for food on the floor while meals or snacks are served, they redirect the children and discard fallen food. An additional concern noted in the review of attendance records from December 15, 2025, to December 19, 2025, were instances of staff-to-child ratios and staff memb ers who are not qualified through DEEDS Early Learning working alone with children on multiple occasions. Attendance records reflected seven total instances of classrooms either being over the staff-to-child ratio or having only staff members with no DEEDS Early Learning qualifications present with children. Staff members with no DEEDS Early Learning qualification were signed in and alone with two-year-old children for the entire week.
Jan 9, 2026OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on December 23, 2025, regarding concerns of unnamed staff not using appropriate behavior management techniques. Eight staff members were interviewed separately and stated they have only observed appropriate positive behavior management within the facility and stated they only use appropriate behavior management support.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
