The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Nov 10, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing (OCCL) received a complaint on September 11, 2025, regarding a lack of supervision, vaping, improper staff-to-child ratio, lack of positive behavior support, not being respectful, and using drugs and being under the influence while caring for children. An investigation was conducted on September 18, 2025. There is no evidence to substantiate this complaint because all staff members interviewed denied the allegations and stated they have never seen staff members sleeping while children were sleeping, or vaping or getting high. Staff members who were interviewed stated they have never seen staff hiding to avoid parents or yelling at each other in front of children. Staff members stated they have never seen a staff tapping a child in the head to get them to walk. |
| Oct 29, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On October 10, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding staff communication, diapering procedures, and not providing physical care routines such as toileting. An investigation was conducted on October 14, 2025. There is no evidence to substantiate this complaint because according to staff interviews and written statements, Child #1 s diaper was checked at least every hour and changed as needed. Staff interviewed stated Child #1 did not always express interest in sitting on the toilet, but they would still try if he/she was willing. Additionally, when Parent #1 came into the center to speak to Administrator #2, he/she was conducting an orientation for a new family and could not speak to him/her right away. |
| Apr 17, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On March 19, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, cleanliness, and accident/injury procedure. An investigation was conducted on March 25, 2025. There is no evidence to substantiate this complaint because Administrator #1 stated Parent #1 was immediately notified of the bite and was given a copy of the accident/ injury report during pick up. This signed report was viewed. During the visit, the center did not smell, and all classrooms complied with the supervision of children. |
| Feb 26, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing received a complaint on January 30, 2024, regarding improper discipline. An investigation began on February 2, 2024, and concluded on February 15, 2024. There is no evidence to substantiate this complaint for improper discipline because the administrator and Staff #1 stated the thinking chair is not time-out. It is voluntary and a way for the children to regroup themselves without time limits put into place by staff. Staff #1 denied grabbing Child #1 in the lower back and stated she grabbed Child #1 by the loop of his pants, while he was crawling under the chair, she was sitting in to prevent him from hurting himself. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
