What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Mar 17, 2026OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint regarding the center not being kept in a clean and safe condition. An investigation was conducted on February 2, 2026. There is no evidence to substantiate this complaint because Staff#1 stated that no children were present in the classroom when they found out the carpeting become wet. Additionally, Staff f#1 stated the children only returned to the classroom when the flooring and carpeting was replaced. During the visit, LS #1 observed new flooring and carpeting in the classroom, and no mold was observed.
Sep 23, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on July 29, 2025, regarding not following diapering procedures and not providing age-appropriate activities. An investigation was conducted on August 7, 2025. There is no evidence to substantiate the allegations in this complaint because staff denied not following diapering procedures and the specialists observed an abundance of age-appropriate activities and materials in the classrooms.
Sep 10, 2024OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint on, August 8, 2024, regarding, improper staff-to-child ratio, interns alone with infants, positive behavior supports, and adult health requirements. An investigation was conducted on August 15, 2024. This complaint is substantiated for interns being alone with infants because according to attendance records, Staff #4, an intern, was alone with infants on July 15, 2024, from 12:02 pm until 1:02 pm, on July 23, 2024, from 7:48 am until 8:19 am, and from 11:41 am to 1:00 pm. There is no evidence to substantiate the other allegations based on staff interviews and a review of attendance records.
Aug 1, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on, June 21, 2024, regarding diapering and failure to provide medication. An investigation was conducted on June 24, 2024. There is no evidence to substantiate the allegations in this complaint because the administrator provided documentation of diaper changes as required, the administrator stated the parent took the inhaler home so the center could not administer it, and the administrator and staff deny applying diaper rash cream to Child #1.
Jul 31, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on June 11, 2024, regarding positive behavior supports, not having an understanding of and respect for the child s needs and culture, not being respectful, and not following their suspension/expulsion policy. An investigation was conducted on June 20, 2024. There is no evidence to substantiate this complaint based on observations and staff interviews.
Mar 13, 2024OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint on February 20, 2024, regarding the center giving out or allowing the use of a child s or family s information without written permission from the parent or guardian. An investigation was conducted on February 22, 2024. This complaint is substantiated because the owner stated the allegations did occur. A concern noted was during the visit the licensing specialists observed a child run from the restroom back to the classroom. When asked, the owner stated Staff #2 was supervising the child going to the bathroom, and the owner was present in the school-age classroom speaking with the licensing specialists when the child exited the bathroom and ran to his classroom.
Nov 8, 2023OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on October 10, 2023, regarding the center s termination policy and staff communication. An investigation was conducted on October 11, 2023. There is no evidence to substantiate this complaint because the center s termination policy states the center will terminate for lack of written communication between the center and the parent and the owner stated the child was not present for one week without communication from the parent. Also, there is no evidence to substantiate the allegation of improper staff communication because all staff denied this.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
