The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

|---|---|---|
| Sep 23, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On September 2, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns involving an injury to Child #1’s eye while in care, bathroom supervision, parent notification of a child injury, and proper injury documentation. An investigation was conducted on September 17, 2026. Based on the investigation, there is no evidence to substantiate the allegation involving inadequate supervision. Staff #1 reported being positioned directly behind Child #1 when the injury occurred and witnessed Child #1 lose balance while reaching for the faucet after a diaper change. Staff #1 stated Child #1 was never alone in the bathroom. Classroom attendance records confirmed two staff members and seven children were assigned to the classroom during the relevant timeframe. Staff #1 consistently reported Child #1 was not attempting to use the toilet when the injury occurred. Child #1 had been taken into the bathroom for a diaper change and was washing their hands afterward. This information is consistent with Child #1 not being toilet trained and addresses the Reporter’s concern regarding why Child #1 was present in the bathroom. LS #1 inspected the bathroom and observed the sink identified by Staff #1. The sink was accessible to children and had rounded ceramic edges. LS #1 did not observe sharp edges, protruding fixtures, a slippery floor, or other environmental hazards capable of establishing an unsafe bathroom condition. Administrator #1 reported a rubber mat was added beneath the sink following the incident as an additional safety measure. There is no evidence to substantiate the allegation involving failure to notify Parent #1 of the injury. According to interviews with Administrator #1 and Staff #1, the injury occurred shortly before Parent #1’s anticipated arrival, and Parent #1 arrived while Staff #1 was providing first aid. Parent #1 was informed of the incident during pick-up. LS #1 also reviewed electronic documentation confirming injury information was entered into ProCare at 3:23 p.m. on September 2, 2026. There is no evidence to substantiate the allegation involving failure to complete or provide injury documentation. Staff #1 completed an injury report and entered electronic notification into ProCare on the date of the incident. The written injury report was provided to Parent #1 the following day and contained Parent #1’s signature. |
| May 14, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On April 7, 2025, the Office of Child Care Licensing (OCCL) received a complaint alleging the overuse of restraining equipment, such as a high chair, with a preschool-aged child. An investigation was conducted on April 28, 2025. Based on the findings of the investigation, there is no evidence to substantiate this complaint. Administrator #1 denied the allegation of excessive restraint and provided information regarding the specific circumstances under which Child #1 is placed in a highchair or stroller, noting these devices are used for short, defined periods during mealtimes and whole group activities to support the child s individual needs. The child, who is non-verbal and has met the criteria for an IEP, requires one-on-one assistance during meals and benefits from additional support to participate in group settings. During the visit, LS #1 observed Child #1 actively engaging with peers in the classroom, displaying mobility and responsiveness, including running, smiling, and using non-verbal vocalizations. LS #1 also observed mealtime routines and confirmed Child #1 required individualized support for feeding. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
