The short answer
Record last released by Delaware Office of Child Care Licensing on Nov 4, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Jul 1, 2026 | OCCL Standards Complaint: Substantiated | The Office of Child Care Licensing (OCCL) received a complaint on June 22, 2026, regarding concerns of operating over staff-to-child ratios and not using respectful communication amongst staff. An investigation was conducted on June 26, 2026. This complaint is substantiated for improper staff-to-child ratios based on staff interviews and review of attendance records. The opening classroom operated over the maximum ratio and group size for a room that has three-year-olds present. On June 22, 2026, there were 44 children present; on June 25, 2026, there were 39 children present, and on June 26, 2026, there were 41 children present. Additionally, review of attendance records showed the school-age classroom over staff-to-child ratio with 16 children present with one staff member on June 24, 2026, and June 25, 2026. There is no evidence to substantiate this complaint for not using respectful communication among staff based on interviews conducted. Of seven staff members interviewed, five staff noted no concerns with administrative staff, stating they felt supported and had no concerns. One staff member stated they felt more support from one administrator over another, and another staff member stated they had a previous incident where they felt Administrator #1 was hostile, but it has since been resolved. An additional concern noted during this investigation was Staff #8 working alone in the four-year-old classroom during nap time without a DEEDS Early Learning certificate. Another concern noted is in addition to exceeding maximum staff-to-child ratio and maximum group size in the opening classroom, the facility also exceeded the maximum capacity for the measured space. This classroom is measured for only 18 children but had up to 44 children at one time based on attendance records and interviews. |
| May 22, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On April 23, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of improper discipline due to threatening or belittling, inappropriate use of “time-out”, inappropriate tone, and lack of supervision due to staff sitting and speaking to each other instead of watching the children. An investigation was conducted on April 30, 2025. There is no evidence to substantiate this complaint for use of improper discipline. All staff interviewed denied witnessing teachers yelling at children or using an inappropriate tone themselves. Staff #3 stated she has never heard any staff threaten children, and according to Staff #2 and #3, time-out is only used as a last resort, and children are in time-out for no longer than five minutes. The children in the classroom are four and five years old; therefore, the length of the time-out is appropriate. There is no evidence to substantiate this complaint for lack of supervision based on interviews and observation of the classroom. All staff interviewed stated they are never sitting down in the classroom and monitor the children by using proximity. Staff #2 and #3 were observed moving around the classroom and engaging with the children. It was noted Parent #1, who is an employee of the center, works in the infant classroom, and Child #1 will often get upset and want to stay with Parent #1. Staff #2 would tell Child #1, You don t want to go to the baby room. |
| Dec 10, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On October 30, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of safety and sanitation due to continued illness, parent complaints/concerns being dismissed, staff and administration not using respectful communication, not having organized staff communication regarding important child info, not having an organized business management system causing child’s belongings to be lost or wrong items to be used, and not providing access to drinking water to children. An investigation was conducted on November 7, 2024. There is no evidence to substantiate any allegations in this complaint. All staff confirmed there was Respiratory Syncytial Virus (RSV) in the one-year-old classroom, but parents were notified, and a notice was posted on the doors of the classrooms. All staff interviewed stated that cots, toys, mirrors, tables, and floors are cleaned daily with soap and water and bleach and water. All classrooms appeared clean and did not have a musty smell during this visit. All staff interviewed stated Staff #1 is not unprofessional or dismissive towards parents. All staff interviewed stated the parents and teachers communicate using the ProCare application, including the children s meals, naps, and if items have gone missing. OCCL does not require a full daily log for one-year-old children, but this facility does enter information into the application for one-year-olds. The staff interviewed also stated children are not denied water, noting each child has their own bottle and it is filled as needed during the day. These bottles were observed during this visit. There is no evidence to substantiate not having an organized system for staff communication and business management because while a child s shoes did get misplaced, Child #2 and #3 had the same pair of shoes and Child #3 was accidently sent home with Child #2 s shoes. Staff interviewed stated Parent #2 was contacted immediately and returned the shoes the next day. Staff int erviewed stated Child #4 ran out of diapers and the center had to use another child s diaper in the meantime. Staff noted Parent #4 was notified of this via the ProCare application, which was seen during this investigation. |
| Aug 16, 2024 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | The Office of Child Care Licensing received a complaint on July 26, 2024, regarding concerns of lack of supervision due to leaving six children unattended and not notifying a parent or providing an accident/injury report about a head injury. An investigation was conducted on July 29, 2024. There is no evidence to substantiate this complaint because interviews with staff members and a review of video footage show Staff #2 did not leave the room unattended and notification to a parent did occur with an accident injury report being written and signed by the parent at departure time. A concern noted at this visit were the children s and staff attendance records did not accurately reflect the staff and children present in the classroom. |
| Jun 12, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On May 22, 2024, and June 4, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of an unexplained injury with no injury report, lack of respectful communication with a parent, and not following the center’s termination policies or policies on handling a parent s complaint. An investigation was conducted on May 31, 2024. Based on the results of this investigation there is no evidence to substantiate Child #1 s parents were not provided an injury report because all three staff who were interviewed stated they never observed marks on Child #1 s arm. There is also no evidence to substantiate a lack of respectful communication because Staff #1 denied being rude to Parent #1 and hanging up the phone. There is no evidence to substantiate the center did not follow its policy on termination of care or handling parent complaints because all three staff who were interviewed stated Parent #1 and Parent #2 were notified care was being terminated because they screamed at Staff #1 and used vulgar language in front of children and other parents. The center s parent handbook states termination of care can occur when children or families participate in disruptive or dangerous behavior. |
| Jun 7, 2024 | OCCL Standards Complaint: Substantiated | On May 10, 2024, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 10, 2024, and accepted it for investigation regarding improper discipline, failure to report suspected abuse or neglect, failure to notify parents of a significant incident to a child, and not following termination policies. An investigation was conducted on May 21, 2024. This complaint is substantiated for improper discipline because five of the staff members interviewed stated they had witnessed Staff #2 improperly discipline children through verbal threats as well as physical contact and provided multiple detailed instances. This complaint is substantiated for failure to report suspected abuse or neglect because multiple staff stated they had witnessed improper discipline by Staff #2 and did not report the incidents to the Child Abuse and Neglect Report Line. Multiple staff also stated they had brought their concerns regarding Staff #2 improperly disciplining children to Administrator Erin Goldberg and Ms. Goldberg did not report the incidents to the Child Abuse and Neglect Report Line. This complaint is substantiated for failure to notify parents of a significant incident to a child because Ms. Goldberg stated she did not contact Child #1 or Child #2 s parent or guardian about the incidents because Staff #1 or Staff #2 was not present at the center on the day of the allegations, and she could not prove anything. However, the attendance documentation collected at the visit shows Staff #1 and Staff #2 were both present and working in classrooms on the morning of the allegations. This complaint is unsubstantiated for not following employment termination policies because Staff #1 was on her documented final warning and was terminated due to her aggressive behavior towards other staff members. Both incident reports regarding Staff #1 s term ination were collected at the visit. |
| Apr 16, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On March 27, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of staff using a drug during the work day that could adversely affect job functions. An investigation was conducted on April 9, 2024. There is no evidence to substantiate this complaint because all staff members interviewed denied witnessing, seeing, or smelling marijuana in the facility or parking lot. The parking lot was observed during the investigation during the time staff take lunch breaks, and no staff members were witnessed smoking in their cars. The facility did not smell of marijuana upon entry. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
