What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Mar 18, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn February 9, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect staff-to-child ratio and staff using cell phones while caring for children. An investigation began on February 12, 2026, and concluded on March 6, 2026. Based on observations, interviews conducted, video footage, and documents reviewed, there is no evidence to substantiate this complaint for incorrect staff-to-child ratio. Based on observations, interviews conducted, and documents reviewed there is no evidence to substantiate for staff using cell phones while caring for children.
May 14, 2025OCCL Standards Complaint: No Evidence to SubstantiateOn April 15, 2025, the Office of Child Care Licensing received a complaint regarding a lack of supervision, not being truthful to parents, not following diaper changing procedures, not providing an accident/injury report, unqualified staff, and not following their policy for notifying parents of an accident/injury. An investigation was conducted on April 23, 2025. Based on observations and interviews during the visit and a review of video footage, there is no evidence to substantiate this complaint for a lack of supervision because both staff were on the playground and engaged with the children. When a child fell, the video shows a staff member responded to the child within two seconds. Based on interviews conducted during the visit, there is no evidence to substantiate this complaint for not being truthful to parents because all staff interviewed stated they did not see Child #1 fall, and they believed the blotches were caused by Child #1’s face touching the mulch. The center called the parent back once they found out Child #1 had fallen and shared the video footage with the parent. Based on interviews conducted at the visit, there is no evidence to substantiate this complaint for not following diaper changing procedures because all staff interviewed knew the correct procedure, and all diaper changing areas met DELACARE requirements. Based on interviews conducted and documentation reviewed at the visit, there is no evidence to substantiate this complaint for not providing an accident/injury report because the parent was notified by telephone, by the app the center uses, and the center provided written documentation to the parent, but the parent refused to sign the accident/injury report. Based on interviews conducted and documentation reviewed during the visit, there is no evidence to substantiate this complaint for unqualified staff. LS #1 reviewed staff files and observed Staff #3 is determined eligible through a criminal background check and is never alone with children. Based on interviews conducted during the visit, there is no evidence to substantiate this complaint for not following their policy for notifying parents of an accident/injury, because the center contacted the parent once they noticed the blotches on Child #1 s face and then called again after further information was gathered and they realized Child #1 fell from the play structure.
Dec 4, 2024OCCL Standards Complaint: SubstantiatedOn November 15, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of unqualified staff alone with children, unsafe playground conditions, and unsafe indoor conditions. An investigation was conducted on November 18, 2024. This complaint is substantiated for unqualified staff alone in the infant room because Staff #1, who is an early childhood intern, was in the infant room alone and is not qualified to be alone with infants. There is no evidence to substantiate this complaint for unsafe conditions on the playground or unsafe conditions indoors. All interior doors were observed to be in proper working order and no broken or unsafe equipment was observed on the playground. A concern noted at the visit was sleep-safe practices were not being followed because an infant was observed with a blanket in their crib.
Jul 31, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn July 12, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of exceeding the maximum group size and combining toddlers with older children. Based on observations, interviews, and attendance records reviewed, there is no evidence to substantiate this complaint for exceeding the maximum group size or combining toddlers with older children.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
