What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Aug 7, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernOn June 22, 2026, the Office of Child Care Licensing (OCCL) received a complaint alleging that staff were not being truthful, fire drills were not being conducted, morning snack was not being provided to one-year-old children, food was not being appropriately cut into small pieces, children were not receiving enough milk or water, and diapers were not being changed frequently. The complaint also alleged that infants were being passed to staff in other classrooms, children were being moved between classrooms, and staff was not adequately supervising children on the playground. Additionally, it was reported that infants were placed in high chairs for more than 30 minutes before breakfast, infant feeding schedules were not being followed, children’s hands were not being washed, toys were not being sanitized, and staff were working while sick. An investigation was conducted on June 29, 2026. There is no evidence to substantiate the allegations in the complaint. Staff members interviewed consistently denied the allegations. Additionally, during the investigation, LS #1 observed staff washing children’s hands after diaper changes, children with their water bottles, and staff offering water to children. LS #1 also observed lunch being served directly from the kitchen, staff assisting children by cutting their food into appropriate-sized pieces, and infants who were developmentally able to feed themselves being placed in high chairs approximately 10 minutes before lunch was served. During the investigation, LS #1 identified one concern unrelated to the allegations. LS #1 observed two staff members escorting children to the community playground by walking them through a parking lot where vehicles were parked. This concern was addressed with the program during the investigation
Jul 10, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn June 12, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding a lack of positive behavior supports. An investigation was conducted on June 15, 2026. There is no evidence to substantiate this complaint because staff members who were interviewed stated they have never observed a staff member hitting a child. Additionally, two children who are enrolled in the preschool classroom stated the teacher would give them a time-out and call their parents if they did something bad. The children also stated that they have never been hit or observed a teacher hitting other children.
Mar 25, 2026OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio, staff using alcohol or drugs that could adversely affect job functions, the administrator not being present the required time, lack of positive behavior supports, handwashing, and diapering procedures. An investigation began on February 18, 2026, and concluded on February 19, 2026. This complaint is substantiated because, according to the administrator s attendance record, Owner #1 was not present the required 27.5 hours per week. There is no evidence to substantiate the allegations of improper staff-to-child ratio, staff using alcohol or drugs that could adversely affect job functions, lack of positive behavior supports, handwashing, or diapering procedures based on interviews, observations, and a review of documentation. An additional concern noted during this investigation was children were observed lying on their sleeping equipment without top or bottom covers.
Jan 13, 2026OCCL Standards Complaint: No Evidence to SubstantiateOCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. The Office of Child Care Licensing received a complaint on October 16, 2025, regarding a lack of positive behavior supports. An investigation began on November 12, 2025, and concluded on December 02, 2025. There is no evidence to substantiate this complaint because Staff #2 and Staff #3 stated they never saw Staff #1 pull the arm of any child. Staff #1 was not available to be interviewed; however, video footage and information were reviewed by another agency, and it was determined there were no charges regarding the incident.
Apr 15, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on March 18, 2024, regarding the center not allowing a change in the meal pattern when requested by the guardian. An investigation was conducted on March 26, 2024. There is no evidence to substantiate this complaint because staff stated Child #1 s guardians were encouraged to get a pediatrician’s note giving the center permission to feed Child #1 baby food after she turned one on February 24, 2024. The center received the note on February 29, 2024. Also, the staff member interviewed stated they informed the uncle that Child #1 ate some of the center s food but had trouble eating and she was offered the baby food.
Dec 19, 2023OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing received a complaint on November 17, 2023, regarding improper staff-to-child ratios and not following accident/ injury procedures. An investigation began on December 1, 2023, and concluded on December 11, 2023. There is no evidence to substantiate this complaint for improper staff-to-child ratio and not following the accident/injury procedures because during the visit, proper staff-to-child ratio was observed in each classroom. Additionally, the attendance records reviewed for November 16, 2023, reflected the classrooms were within proper staff-to-child ratio. Also, the staff followed the accident/ injury procedures by contacting the parent regarding the injury immediately, applying ice, and completing the accident/ injury report. The parent, Ms. Rooke, and Staff #2 signed the report.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
