What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Sep 30, 2026OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) received a complaint on August 17, 2026, regarding a lack of supervision. An investigation was conducted on August 19, 2026. This complaint is substantiated because Administrator #1 confirmed that video footage showed Staff #1 leaving a group of children unattended in the classroom during naptime from approximately 12:35 p.m. to 12:38 p.m. on August 13, 2026. Administrator #1 stated Staff #1 left the classroom for a break without confirming that another staff member was present to supervise the children. The children remained unattended until another staff member entered the classroom. Additionally, Staff #1 did not sign out of the classroom on the attendance record.
Aug 12, 2026OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on June 24, 2026, regarding inaccurate attendance records, not having the OCCL required postings, and having a classroom without the required equipment and activity areas. An investigation was conducted on July 8, 2026. There is no evidence to substantiate the complaint because Owner #1 and Staff #1 explained that the multipurpose room was used temporarily while repairs were being completed following damage to the filtration system caused by a burst water pipe. Documentation showed that parents of the affected classrooms were notified of the temporary classroom relocation on May 8, 2026. Staff #1 further stated that staff-to-child ratios were maintained and remained in compliance throughout the temporary relocation and a review of documentation confirmed this. During the complaint visit, LS #1 observed the classroom and found it to be in compliance with licensing regulations.
Mar 25, 2026OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint regarding a lack of positive behavior support. An investigation was conducted on February 18, 2026. There is no evidence to substantiate this complaint because the video footage reviewed did not show Staff #2 hit Child #1 on the cheek or display other inappropriate physical interactions.
Jun 12, 2025IA Investigation: SubstantiatedDelaware gives no summary.
Mar 18, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on January 24, 2025, regarding improper discipline, vaping, and air quality. An investigation began on February 3, 2025, and concluded on February 5, 2025. There is no evidence to substantiate this complaint because, during the visit, LS #1 and LS #2 did not observe any mold in the center, and Owner #1 stated when he found out about the mold, he closed the classroom and hired a company to conduct and remove the mold. There is no evidence to substantiate the allegations of improper discipline and vaping because staff who responded to the interview during the visit denied them. One staff called after the visit and stated two staff were vaping in the bathroom but could not provide any additional details
Mar 18, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on January 24, 2025, regarding vaping/smoking, evidence of rodents or hazardous child care space, sanitation, discrimination, lack of supervision, and respectful interactions with children. An investigation began on February 3, 2025, and concluded on February 5, 2025. There is no evidence to substantiate this complaint because, during the visit, LS #1 and LS #2 did not observe any hazardous conditions, mold, mice, or mice droppings in the center. Additionally, staff members who were interviewed denied the allegations.
Jan 13, 2025OCCL Standards Complaint: SubstantiatedThe Office of Child Care Licensing (OCCL) cannot legally disclose if a complaint was also received or investigated by any other agency. OCCL investigations of regulatory non- compliance begin when clearance from other agencies, when applicable, has been received. The Office of Child Care Licensing (OCCL) received a self-reported complaint on November 18, 2024, regarding improper discipline. An investigation began on November 18, 2024, and concluded on January 8, 2025. This complaint is substantiated for improper discipline because Staff #1 was observed roughly handling Child #1 in the classroom on video footage as reported by the administrator. Staff #1 s employment was terminated by the center after they reviewed the video footage.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
