Made Jan 21, 2016, sent to the company Feb 2, 2016, published by CPSC Feb 18, 2016

The product, in the report: Included in initial write up

A small plastic baby book made by Prestige Toy Corp NY, NY 10001, UPC code 0-20809-05633-6 PA# 16212 (CN) Style 5633 PO# 012670 0814, surface washable Brand is Prestige Baby, the title of the book is Numbers. Total of four pages 3”x3”. Inside the first page (cover) is a small plastic squeak toy, that can be squeezed. The squeak toy was sealed between the pages. The sealed edge has come apart and the squeak device came out. Our 8 month old was chewing on it. This is 100% a choking hazard. The book in of in itself is designed to be chewable. This product should be pulled from the market until such time they can guarantee the page will not come apart or they find another way to put a squeak toy in the book that isn’t small enough to choke on.
