The reports, as CPSC published them

Reports people made to the U.S. Consumer Product Safety Commission. CPSC publishes each one after sending it to the company named, and doesn’t guarantee they’re accurate. A report isn’t a recall.

Each report is the words of the person who made it, as CPSC published them. The company’s comments, where CPSC published any, are set apart as the company’s own.

The product, in the report: A soft crinkle book version of The Very Hungry Caterpillar with a plastic strawberry attached. It’s this one: [REDACTED]

I gave my 4 month old daughter The Very Hungry Caterpillar soft book to play with, and stepped away for a few minutes. I heard her make a strange noise while in the other room, and came to check on her. She had rolled on top of the hard plastic strawberry attached to the book with a string, and had the string wrapped around her neck, choking her. Fortunately I was able to unwrap it, and she was not hurt, but if I hadn’t heard her, she could have strangled herself.

The product, in the report: Soft (crinkly) baby book titled “A is for Animals” by Kids Preferred.

My 8 month old baby was playing with a soft book (“A is for Animals” by Kids Preferred). The book has a “bookmark”, which is a string with one end attached to the top of the plastic book binding, while the other end has two plastic shapes (one circle, one star) that the baby likes to chew on. My baby was lying on the floor on her back while playing with the book. Suddenly, she ended up with the book stuck under one side of her head and the plastic shapes stuck under her head on the other side, with the string “bookmark” strangling her across her neck. She started turning red in seconds. She screamed and I was able to remove the book before any damage was done, but it became clear to me that the string “bookmark” is a serious strangulation hazard.
