The product, in the report: The product is the Dare-u-go bib in the gray color. The failure was with the included utensil.

My child was using a Dare-u-go bib. It has an included utensil. When my child was trying to use the utensil to get food out of the bib compartment and bent it slightly, it shattered. It created sharp edges and choking hazards. He is 20 months old, so not very strong. He was not injured because we reacted quickly and removed it.
