First aid Made Jun 25, 2018, sent to the company Jul 3, 2018, published by CPSC Jul 18, 2018
The product, in the report: Fisher Price little sailor outfit with black and white pants with 3 pink buttons by ankles of each pant leg, pink shirt with black and white collar, with 3 fish on right side of shirt with the words “ocean friends” and 3 pink buttons on back
Daughter was wearing fisher price 2 piece clothing set with buttons by ankles of pants. She was pulling on pants and the button must have fallen off and she put it in her mouth and began to choke on it. I (parent) was able to dislodge the button and prevent further choking and injury. Upon further inspection, the thread pulled completely out. It was not secured at all. The other buttons are secured in the same way and with a gentle tug of the thread they come apart and the button falls off.
