Product: Cosara So Tight - male masturbation sleeve with electronic suction/vacuum Date of purchase: July 31 Where purchased: Cosara Website What happened that made you want to report? I am reporting a product defect that caused injury and that the manufacturer has acknowledged cannot be fixed or replaced. First incident: August 11, 2025. I used the product with water-based lubricant as directed. I had also taken sildenafil for ED. During use the battery died and the device clutched my penis with strong vacuum/suction and I could not get it off. I sustained an injury. I went to the doctor. At that time the doctor thought it could be related to the medication. Second incident: September 11, 2025, between 2-3 AM. I used the product again following the manufacturer’s instructions - I applied water-based lubricant appropriately and took NO medication, as instructed by the doctor. Again, when the battery died, the device clutched my penis with excessive vacuum and I could not remove it. I reported this to the company. They responded in writing that what I described is “not normal and is a safety concern” and that “the device should never clutch so tightly that it causes pain or feels like it could cause injury.” They also stated “the clutch effect worsens when the battery is low.” They said they do not offer an adjustable version and that the defect cannot be fixed or replaced. They offered a 30% partial refund as a goodwill gesture if I keep the device. I did not accept. I went to the doctor for observation after the second incident. The doctor observed that my penis was injured by the device and told me not to use it again. I am reporting because the battery-failure mode causes the device to tighten and trap, creating a risk of injury, and the manufacturer has no fix. What type of product(s) were being used? Cosara So Tight - electronic suction male masturbation sleeve device. Battery powered, 4 suction levels, with an end cap the model. Product clutches with vacuum pressure. Who was using the product(s)? Adult male consumer, age age group 40-50, sole user of the product. How were the product(s) being used? Product was being used as intended, per manufacturer’s instructions: Water-based lubricant applied generously inside sleeve and on body Device set to varied levels.

Second incident was without any ED medication, per doctor’s instruction, to isolate the cause. In both incidents, the injury occurred when the battery died during use, causing loss of release function and increased clutch/vacuum. Were there any injuries that happened or may have happened while using the product? Yes, injury occurred on both dates. August 11: Pain, difficulty removing device, injury to penis. Evaluated by doctor. September 11: Repeat injury - penis injured by device when battery died, clutching and unable to remove. Pain, bruising/swelling, difficulty removing. Evaluated by doctor after incident who observed injury caused by device. Potential for more severe injury if device cannot be removed promptly - risk of circulation injury. How did any injuries occur? The device uses electronic suction/vacuum to create tightness. When the battery died during use, the vacuum pressure increased or failed to release, causing the sleeve to clutch tightly onto the penis. The device could not be removed immediately. The prolonged tight clutching and vacuum caused pain and injury to the penis. Manufacturer has acknowledged in writing “the clutch effect worsens when the battery is low” and that this level of tightness “should never” happen. This indicates a known failure mode on low/dead battery without a mechanical fail-safe release.
