The product, in the report: AQUEON 10 WATT HEATER 120 VOLT 60HZ FISH AQUARIUM EQUIPMENT 44P4/05174
I bought my 5 year old 3 fish and one alge eater along with and AQUEON 10WATT HEATER. We woke up this morning only to find all 4, which looked to be fried in the water. Luckily I unplugged the heater and light to move the tank which had a rainbow oil coating on the top of water. When I looked at the heater it was bulging out on the bottom front and the left lower side had a hole in it with a thumb print size indent on it as well as the cord was separated with some kind of goldish colored erosion on it.I looked up the item only to find over 20 different videos of people getting electrocuted and there fish were all dead. My daughter could have been electrocuted if I was not there to help her feed her fish. 2 gallon tank. Product numbers are AQ10W......44P4....05174

The product, in the report: Submersible water heater for fishtanks, Aqueon 06107 Pro
Got shocked by “Aqueon 06107 Pro” aquarium water heater today. I briefly put my hand in the fishtank and got a powerful zap through my body. Pulled out a multimeter and measured full 120v between fishtank water and a neutral pin in a nearby outlet. Unplugged fishtank pump, lights, and heater and isolated the cause to the heater.

The product, in the report: Aqueon pro 100w aquarium heater
Under normal use conditions, our Aqueon 100w aquarium heater suddenly cracked along its entirely length and failed catastrophically. Smoke began to billow from the heater, and had we not quickly unplugged it, it likely would have started a fire.

Hurt, no treatment Made Jan 30, 2017, sent to the company Feb 15, 2017, published by CPSC Mar 2, 2017
The product, in the report: Aquarium heater
I purchased a 200 watt Aqueon Pro Fish Tank Heater on June 9, 2014 from Petsolutions (Beavercreek, OH). The heater has at all times been operated according to the manufacturer’s instructions, including strict adherence to the minimum water level. While working in my saltwater aquarium, I received a line voltage electric shock. Connecting a probe to the saltwater and checking it against ground with a voltmeter, I was shocked (pun intended) to learn that substantial voltage was running through the water. I am lucky I was not severely injured. Removing devices, one by one, I isolated the cause to the Aqueon heater. As it was installed in a hang-on back protein skimmer, I could not see that the shell had cracked, exposing the innards of the heater to the saltwater. Looking online, I found at least one other instance wherein something similar may have occurred: https://www.monsterfishkeepers.com/forums/threads/aqueon-pro-heater-shell-cracking.609125/

The product, in the report: Aqueon Mini Bow 1 gallon fish tank, incandescent bulb, dark blue hood/light compartment
I purchased an Aqueon Mini Bow 1 gallon fishtank, new in box, from Petsmart in Alexandria, VA on 1 July 2015. The tank was set up that day with all original products- including the light bulb that came with the tank setup. I used the tank for 5 weeks, without incident. I did note the tank hood would become warm during the day while it was on, but it was not substantially hot or anything that caused concern. On August 11, 2015 I went to feed the fish and found that the tank hood plastic was melted and bubbled. The safety sticker on the inside of the hood prevented the melted plastic from touching the bulb and likely prevented a fire. The safety sticker states to use a 7 watt bulb or less. I am not sure of the wattage of the bulb- it is not marked- but it is the original bulb that was sold with the new tank.

No one hurt Made Jul 7, 2012, sent to the company Jul 16, 2012, published by CPSC Jul 30, 2012
The product, in the report: Aqueon fish tank hood light for 29 gallon. 30” Fluorescent Aquarium Reflector 120 volt 20 watt 60 hz Other numbers are 9n23 and E163201
I own nine fish tanks. Most have aqueon hoods with lights. On 06/24/2012 my husband and I smelled something burning in our living room. I found the smell and smoke directly over my fish tank and unplugged light source. I took apart hood the next day and part number 1105Y6 (the box where all the wires meet) had melted altogether in one clump. The hood plastic piece that this box was inside is labeled: All_Glass Aquarium.,Inc. Franklin, Wisconsin 30” Fluorescent Aquarium Reflector 120 volt 20 watt 60 hz Other numbers are 9n23 and E163201
I have serious concerns about leaving any of my other tank lights on all day anymore, as was my habit for the well being of the fish. So I do not do so anymore. Do cheap products mean we must face the possibility of our houses burning down? I am just feeling very lucky that I was home that day to avert a catastrophe.

No one hurt Made Mar 2, 2012, sent to the company Mar 12, 2012, published by CPSC Mar 26, 2012
The product, in the report: Aqueon Aquarium Water Changer - 25 Feet. ASIN: B000YAJKL6. Item model number: 06089. http://www.amazon.com/gp/product/B000YAJKL6
I purchased the Aqueon Aquarium Water Changer (25 Feet Hose) for use with my 2 tanks, 30 and 50 gallons. I have turtles, so there are more electric cords than with a standard aquarium setup. For each tank, I have a heat lamp, uva/uvb lamp, submersible heater, and filter/pump, so there are 8 cords in the room.
The adapter fits the sink fine. It’s easy and doesn’t leak. The hose connects at one end to the sink adapter and at the other end to the vacuum. It’s easy to push it on and get a tight seal.
The product worked amazingly well! But just as the first tank was approaching the fill mark, the hose flew off the sink adapter! It flew in a 3 foot arc, flinging a large amount of water outward in a large area at high velocity. In 1 second, half the room was soaked - including the power strip!! The strip was turned off, but we quickly unplugged it anyway.
In 8 subsequent attempts to reconnect the hose to the adapter, it flew off, slinging water in every direction. This product is going to electrocute someone, or their aquatic pets - if it hasn’t already.
