The short answer

NHTSA received Graco Children’s Products Inc.’s report on Feb 2, 2016. Copied from NHTSA on Oct 19, 2026.

Every line below is in NHTSA’s own words, with its source beside it.

What’s wrong with it?

If the instructions are followed as shown, a rear-facing infant could be placed in an upright position and/or a forward-facing toddler could be placed in a recline position. Either scenario may increase the risk of injury to the child in the event of a crash.

Graco Children’s Products, Inc.

(Graco) is recalling certain Extend2Fit Convertible child seats manufactured November 27, 2015, to January 20, 2016.

The affected child seats may have the recline label affixed at the wrong location, resulting in confusion about how to use the seat properly.

As such, these child seats fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 213, “Child Restraint Systems.”

What the record doesn’t say

  • NHTSA’s recall file doesn’t say whether anyone was hurt.
  • Whether one vehicle or seat has been fixed: only its maker, or NHTSA’s lookup with a VIN, can say.
  • How many have been returned or fixed isn’t in the notice.
