The short answer

NHTSA received Motor Coach Industries’s report on Apr 16, 2013. Copied from NHTSA on Oct 26, 2026.

Every line below is in NHTSA’s own words, with its source beside it.

What’s wrong with it?

During a regeneration process to clean exhaust emissions, if the bellows pipe has failed, the hot exhaust gases may not be contained, resulting in a fire.

Motor Coach Industries (MCI) is recalling certain model year 2012-2013 D4000, D4000ISTV, D4005, D4500, and D4505 motorcoaches manufactured from May 2012 through December 2012 and equipped with a Cummins ISX EPA 2010 engine.

In the affected buses, the bellows pipe in the exhaust system may have been incorrectly assembled, which could result in a failure of the pipe during operation.

What the record doesn’t say

  • NHTSA’s recall file doesn’t say whether anyone was hurt.
  • Whether one vehicle or seat has been fixed: only its maker, or NHTSA’s lookup with a VIN, can say.
  • This recall names only model years before 2015, which have no pages here. Your VIN, at NHTSA, says which recalls are open on a vehicle.
  • How many have been returned or fixed isn’t in the notice.
