Every line below is in NHTSA’s own words, with its source beside it.
What’s wrong with it?
WHEN THE CYLINDER IS EXTENDED TO ITS MAXIMUM STROKE, THE FIFTH STAGE CAN SEPARATE FROM THE CYLINDER ASSEMBLY CAUSING LOSS OF CONTROL OVER THE TRAILER, ALLOWING THE TRAILER TO FREE FALL, POSSIBLY CAUSING SERIOUS INJURY OR DEATH.
ON CERTAIN END DUMP TRAILERS, THE 8-INCH FIVE STAGE TELESCOPIC CYLINDER COMMONLY REFERRED TO AS A “SUPER DUMP”, WITH USE THE PACKING NUT ON THE FOURTH STAGE RETAINING THE PLUNGER CAN LOOSEN, BACKING OFF THE FOURTH STAGE.
THIS CONDITION, IF NOT REMEDIED, CAN CONTINUE UNTIL THE PACKING NUT BACKS COMPLETELY OFF THE STAGE.
IF THIS OCCURS, THERE IS NOTHING TO RETAIN THE FIFTH STAGE (PLUNGER) IN THE ASSEMBLY.

What the record doesn’t say
- NHTSA’s recall file doesn’t say whether anyone was hurt.
- Whether one vehicle or seat has been fixed: only its maker, or NHTSA’s lookup with a VIN, can say.
- This recall names only model years before 2015, which have no pages here. Your VIN, at NHTSA, says which recalls are open on a vehicle.
- How many have been returned or fixed isn’t in the notice.
