The short answer

NHTSA received TOYOTA MOTOR NORTH AMERICA, INC.’s report on Oct 6, 2003. Copied from NHTSA on Oct 20, 2026.

Every line below is in NHTSA’s own words, with its source beside it.

What’s wrong with it?

IN THE WORST CASE, THE STUD BOLT(S) MAY FRACTURE, WHICH MAY RESULT IN THE SEPARATION OF THE WHEEL FROM THE HUB ASSEMBLY AND LEAD TO A LOSS OF VEHICLE CONTROL.

ON CERTAIN PASSENGER VEHICLES EQUIPPED WITH FACTORY ALUMINUM ALLOY WHEELS, THE LUG NUT(S) MAY NOT SECURE THE WHEEL PROPERLY TO THE HUB ASSEMBLY DUE TO OVER-DRILLING OF ONE OR MORE OF THE FIVE LUG NUT HOLES ON THE ALLOY WHEEL.

IF THE VEHICLE IS DRIVEN FOR AN EXTENDED PERIOD IN THIS CONDITION, THE LUG NUT(S) COULD POTENTIALLY LOOSEN.

What the record doesn’t say

  • NHTSA’s recall file doesn’t say whether anyone was hurt.
  • Whether one vehicle or seat has been fixed: only its maker, or NHTSA’s lookup with a VIN, can say.
  • This recall names only model years before 2015, which have no pages here. Your VIN, at NHTSA, says which recalls are open on a vehicle.
  • How many have been returned or fixed isn’t in the notice.
