The short answer

NHTSA received PIERCE MANUFACTURING,INC.’s report on Apr 2, 2001. Copied from NHTSA on Oct 20, 2026.

Every line below is in NHTSA’s own words, with its source beside it.

What’s wrong with it?

UNDER THESE CONDITIONS, THE DRIVER CAN EXPERIENCE UNINTENDED TURNING OF A SINGLE OR A TANDUM REAR AXLE, POSSIBLY LEADING TO A VEHICLE CRASH OR INJURY OR DEATH TO A PEDESTRIAN OR FIRE FIGHTER.

VEHICLE DESCRIPTION: FIRE TRUCKS EQUIPPED WITH OSHKOSH ALL-STEER ALL-WHEEL STEERING SYSTEM.

THE REAR WHEELS OF A FIRE ENGINE CAN TURN UNINTENTIONALLY IF THE ALL-WHEEL STEERING SYSTEM KEY IS ON AND MOISTURE PENETRATES TO THE WIRE THAT CONNECTS THE STEERING SENSOR TO THE ELECTRONIC CONTROL UNIT (ECU).

IT WAS DISCOVERED THAT THE INSULATION COVERING THE WIRES BETWEEN THE SENSOR AND THE ECU HAD BEEN NICKED DURING TIME OF ASSEMBLY.

OVER TIME, THE NICK IN THE INSULATION ALLOWED MOISTURE TO PENETRATE TO THE ACTUAL WIRE.

What the record doesn’t say

  • NHTSA’s recall file doesn’t say whether anyone was hurt.
  • Whether one vehicle or seat has been fixed: only its maker, or NHTSA’s lookup with a VIN, can say.
  • This recall names only model years before 2015, which have no pages here. Your VIN, at NHTSA, says which recalls are open on a vehicle.
  • How many have been returned or fixed isn’t in the notice.
