The short answer

NHTSA received AM SAFE, INC.’s report on Feb 20, 2001. Copied from NHTSA on Oct 20, 2026.

Every line below is in NHTSA’s own words, with its source beside it.

What’s wrong with it?

SERIOUS INJURY OR DEATH COULD OCCUR IF THE VEHICLE OCCUPANT IS NOT PROPERLY RESTRAINED.

EQUIPMENT DESCRIPTION: CENTER PUSH BUTTON SEAT BELT BUCKLE CONNECTORS (TONGUES) FROM LOT 9909 AND ASSEMBLIES USING THESE CONNECTORS MANUFACTURED BETWEEN SEPTEMBER 1 AND SEPTEMBER 30, 1999.

CONNECTORS FOR THE AFFECTED LOT WERE SENT THROUGH A SECONDARY PROCESS AFTER STAMPING.

THE SECONDARY PROCESS FORMED A CHAMFER IN THE CONTACT SURFACE OF SOME OF THE CONNECTORS.

A CHAMFER IN THE SURFACE WILL ALLOW THE CONNECTOR TO DISENGAGE FROM THE BUCKLE DURING A VEHICLE CRASH LEAVING THE OCCUPANT UNRESTRAINED.

THESE BUCKLE ASSEMBLIES FAIL TO COMPLY WITH FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 209, “SEAT BELT ASSEMBLIES.”

What the record doesn’t say

  • NHTSA’s recall file doesn’t say whether anyone was hurt.
  • Which ones, by part or lot: the recall’s report on NHTSA’s site gives them where the maker did; this file gives part numbers only where its maker listed them.
  • How many have been returned or fixed isn’t in the notice.
