The short answer
- Not rated by Medicare: CMS rates only the providers it certifies, and this one isn’t Medicare-certified
- Not read what inspectors found: the state’s list doesn’t carry it
Each figure’s source sits beside it, in its section below.
What these words mean7
- Not read
- The file this site reads doesn’t carry what inspectors found, so nothing is counted here. It doesn’t mean the place has a clean record: the state’s own site may have its reports. Source: How this site reads each source.
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Scope and severity (A to L)
- CMS grades each nursing home deficiency with a letter. A to C: no actual harm, with potential for minimal harm. D to F: no actual harm, but potential for more. G to I: actual harm. J to L: immediate jeopardy. Within each band, the letter rises as more residents were affected or could be: one or a few, a pattern, or widespread. Source: CMS, nursing home scope and severity grid (State Operations Manual, chapter 7).
- Nurse staffing hours
- The hours registered nurses, licensed practical nurses and nurse aides worked, per resident per day, from the payroll records each home sends CMS. Source: CMS, Payroll-Based Journal.
- Fines and payment denials
- A fine (civil money penalty) is money CMS makes a nursing home pay when it falls short of federal rules. A payment denial means Medicare and Medicaid stop paying for new admissions until the home meets the rules again. Source: CMS; 42 CFR 488.430 and 488.417.
What did inspectors find?
- Michigan’s map layer carries no inspection results, so this page can’t say what inspectors found. LARA posts each facility’s surveys on its own search, whose site terms don’t allow automated reading without a written agreement.
Is it licensed, or certified?
- Nursing home, licensed by Michigan’s Department of Licensing and Regulatory Affairs.
- Michigan’s licence list marks no certification, and CMS’s files of the providers Medicare certifies (Care Compare and the Provider of Services file) don’t hold it by its name in its city or its street, so it isn’t Medicare-certified. CMS’s files list only the nursing homes Medicare certifies, so no CMS CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System., survey or inspection covers this one, and none is shown or compared.
- Michigan’s licence number: 1070000123.

Has the state acted against it?
- Michigan’s map layer carries no enforcement actions; LARA’s own search has them.
Who does the caring?
- Michigan publishes no staffing levels in this list, and CMS’s staffing measures cover only certified nursing homes.
Who owns it?
- This list doesn’t name owners.
How do residents fare?
- CMS rates only Medicare-certified nursing homes, so no CMS rating is shown here; Michigan publishes no rating in this list.
How does it compare nearby?
- Not compared: CMS’s measures cover only the nursing homes Medicare certifies, and this state’s list publishes no measure of its own to compare.
What the record doesn’t say
- Michigan licenses every nursing home, and LARA’s map layer lists each licence, but not which Medicare certifies. This page is one CMS’s files don’t hold by its name, its street or its number, and that a check row by row found licensed only: so no CMS rating or inspection covers it.
- CMS’s files list only the nursing homes Medicare certifies, so no CMS star rating, survey or inspection covers this one, and none is shown or compared.
- Michigan’s map layer carries no inspection results, so this page can’t say what inspectors found. LARA posts each facility’s surveys on its own search, whose site terms don’t allow automated reading without a written agreement.
- LARA’s map layer gives no licence dates or status, only the licences in force.
