What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Prevention (42 CFR 418.60(a))

    The inspectors’ words, 219 words

“It is required that the hospice follows accepted standards of practice to prevent the transmission of infections and communicable disease, including the use of standard precautions, such as hand hygiene. This standard was not met as evidenced by document review, interview and observation. In 2 of 2 (100%) home visit observations revealed the agency did not ensure the staff followed the hand hygiene policy. DOCUMENT 2: Agency Policy: Hand Hygiene (xxx) 041321 Hospice – (xxxx) stated in part, “Decontaminate hands after contact with a client’s intact skin (e.g., when taking a pulse or blood pressure, and lifting a client. Decontaminate hands after contact with body fluids or excretions, mucous membranes, non-intact skin, and wound dressings if hands are not visibly soiled. Decontaminate hands after removing gloves and between glove changes.” HOME VISIT 1: During the home visit (Clinical Record 1) on 02/19/2025 with the Registered Nurse (RN) (Personnel Record 6), the RN was observed changing the gloves without decontaminating the hands. HOME VISIT 2: During the home visit (Clinical Record 2) on 02/19/2025 with the Hospice Aide (HA) (Personnel Record 9), the HA was observed changing the gloves during patient care without decontaminating the hands. During a joint document review and interview on 02/19/2025, the Alternate Director of Nursing agreed the staff did not follow the hand hygiene policy.”

    Rule L0579. Standard-level.

  • Hospice aide assignments and duties (42 CFR 418.76(g)(2)(ii))

    The inspectors’ words, 153 words

“It is required that a hospice aide provide services that are included in the plan of care. This standard was not met as evidenced by clinical record review, interview and observation. In 1 of 2 (50%) clinical records reviewed of patients receiving hospice aide services revealed the agency did not ensure that the hospice aide provided services included in the plan of care. HOME VISIT 2: The Hospice Aide Plan of Care dated 02/14/2025 documented Bed Bath every visit. During the home visit (Clinical Record 2) on 02/19/2025 with the Hospice Aide (HA) (Personnel Record 9), the HA was observed giving the patient a shower using a shower chair. The Hospice Aide Visit note dated 02/19/2025 documented “completed” for Bed Bath every visit. During a joint clinical record review and interview on 02/20/2025, the Alternate Director of Nursing confirmed that the hospice aide did not provide services included in the plan of care.”

    Rule L0626. Standard-level.

  • Hospice aide assignments and duties (42 CFR 418.76(g)(4))

    The inspectors’ words, 218 words

“It is required that hospice aides must report changes in the patient’s nursing needs to a registered nurse as the changes relate to the plan of care and any quality assessment and improvement activities. The hospice aide must also complete appropriate records of service, including the report of changes in the patient’s needs. Hospice aide documentation complies with hospice policies and procedures. This standard was not met as evidenced by clinical record review and interview. In 1 of 2 (50%) clinical records with hospice aide services reviewed revealed that the agency did not ensure the hospice aide notified the registered nurse of the changes in the patient’s plan of care. CLINICAL RECORD 2: The Hospice Aide Plan of Care dated 02/14/2025 documented Bed Bath every visit. The documentation lacked notifying the registered nurse of changes to the plan of care. During a joint clinical record review and interview on 02/20/2025, the Alternate Director of Nursing confirmed that the hospice aide documentation lacked notification to the registered nurse of the changes in plan of care.”
