What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Nov 21, 2024: recertification inspection by the state2 deficiencies
  • Content of plan of care (42 CFR 418.56(c))

    The inspectors’ words, 209 words

“Based on clinical record review, the hospice failed to ensure in two of nineteen clinical records reviewed (Patient # 5 and 12) a complete and comprehensive physician Plan of Care (POC) was developed to meet the needs of the patient. Failure to develop a complete and comprehensive POC has the potential to result in unmet patient needs and negative patient outcomes. Patient # 5 (start of care 08/23/2024) has diagnoses of Chronic Obstructive Pulmonary Disease, Unspecified and Chronic Respiratory Failure with Hypoxia. The physician’s Plan of Care (POC) for certification period 08/23/2024 through 11/20/2024 contains as order for Azithromycin 250 milligrams (mg) oral tablet; take one tablet orally three times a week. The order is incomplete as it does not state the time interval between doses. Patient # 12 (start of care 10/25/2024) has diagnoses of Intrahepatic Bile Duct Carcinoma and Personal History of other Venous Thrombosis and Embolism. The POC for certification period 10/25/2024 through 01/22/2025 contains an order for Enoxaparin 40 mg/0.4 milliliters (ml) injectable solution; inject 0.4 ml subcutaneously every twelve hours. The POC is incomplete as it lacks bleeding precautions. The findings were confirmed on 11/19/2024 at 3:45 PM and 11/21/2024 at 1:55 PM with the Director of Patient Services; no additional evidence was provided.”

    Rule L0545. Standard-level.

  • Control (42 CFR 418.60(b)(2))

    The inspectors’ words, 118 words

“Based on review of the hospice’s infection control policies and procedures evidence is lacking the hospice has ensured the development of a comprehensive infection control program that protects patients, families, visitors, and hospice personnel by preventing and controlling infections. Failure to develop a comprehensive infection control program has the potential to place patients and staff at risk for exposure to infectious diseases which may result in negative patient outcomes and adversely affect hospice operations. The hospice’s policy and procedure entitled, “Infection Control Program”, policy number ADM-P029.05, was reviewed. The policy lacks infection prevention protocols when performing wound care. The finding was confirmed on 11/18/2024 at 1:25 PM with the Director of Patient Services; no additional evidence was provided.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
