What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 222 words
“It is required that the hospice follow accepted standards of practice to prevent the transmission of infections and communicable disease, including the use of standard precautions, such as hand hygiene and use of gloves, mask, eye protection, or face shield depending on anticipated exposure. This standard was not met as evidenced by document review, interview and observation. DOCUMENT 1: Policy No. 7-009 Hand Hygiene policy dated 2024 read in part “Hand decontamination using an alcohol-based rub should be performed: E. When moving from a contaminated body site to a clean body site during patient care, G. After removing gloves.” HOME VISIT 1: The patient elected hospice care on 06/06/2024 with a diagnosis of Alzheimer’s Disease. During the home visit with the Hospice Aide (HA) [Personnel Record 1] on 03/27/2025, the HA provided pericare to the patient and then cleaned the patient’s posterior legs, changed gloves, and completed the assigned tasks (dressed the patient and provided oral care). The HA did not perform hand hygiene before moving from a contaminated body site to a clean one during patient care or after changing gloves. During an interview on 03/27/2025, the Director of Clinical Services [Personnel Record 5] confirmed that the HA should have cleansed hands before moving from a contaminated body site to a clean body site during patient care and after removing gloves.””

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
