What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Review of the plan of care (42 CFR 418.56(d))
The inspectors’ words, 360 words
“Based on record review, policy review, and staff interview, the Hospice failed to update the plan of care for 1 of 10 sampled patients (Patient #1) currently receiving Hospice services. Failure to review and revise the plan of care with information from the patient’s updated comprehensive assessment limited the staff’s ability to communicate treatment approaches and interventions to ensure continuity of care. Findings include:Review of the agency policy titled “Volunteer Program” occurred on 05/29/25. This policy, dated December 2010, stated, “. . . Policy: . . . to utilize the volunteer as an integral part of the interdisciplinary group. Procedure: . . . It is the intention of hospice to utilize volunteers to supplement the care being provided . . .”Review of Patient #1’s medical record for the current certification period of 03/09/25 to 06/06/25 occurred on May 28-29, 2025. Diagnoses included malignant tumors, dementia, and anxiety. The current plan of care contained an order for “Hospice medical social worker to evaluate . . . need for additional care/resources, adjustment to care and develop a plan of care. . . .” The Interdisciplinary Group (IDG) Comprehensive Assessment and Plan of Care Update Report, for the IDG meeting dated 03/12/25, stated, “. . . Current Meeting Summary: . . . Social Worker: . . . Volunteer Needs: Volunteer requested for socialization. . . .” Hospice staff held an IDG meeting every two weeks from March 12 to May 21 but the meeting summary lacked an update to the use of volunteers.Review of volunteer progress notes showed a volunteer visited Patient #1 on eight occasions between 01/27/25 and 3/6/25 and provided the service of companionship. The record contained no volunteer progress notes for the current certification period.During an interview on 05/28/25 at 4:50 p.m., the volunteer coordinator (#1) stated the volunteer was no longer seeing Patient #1 due to personal family issues but failed to notify her until today. She planned to assign another volunteer.During an interview on 05/29/25 at 10:15 a.m., an administrative staff member (#2) stated information regarding volunteer services and frequency of visits “should be on the plan of care” and agreed Patient #1’s care plan lacked this information.”

- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
