A hospice in Brooklyn Center, Minnesota 55429
Also listedWith Minnesota’s licensed hospices Medicare doesn’t certify
Besides the hospices CMS certifies, Minnesota’s list here holds the hospice providers the Department of Health licenses that its data gives no Medicare number. CMS’s ratings and inspections don’t cover them; each says so, and is compared with none.

The short answer
- Not rated by Medicare: CMS rates only the providers it certifies, and this one isn’t Medicare-certified
- Not read what inspectors found: the state’s list doesn’t carry it
Each figure’s source sits beside it, in its section below.
What these words mean3
- Not read
- The file this site reads doesn’t carry what inspectors found, so nothing is counted here. It doesn’t mean the place has a clean record: the state’s own site may have its reports. Source: How this site reads each source.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
What did inspectors find?
Minnesota’s provider data carries no inspection results, so this page can’t say what inspectors found. The Department of Health says its Health Care Provider Evaluation and Investigation Results search “is designed to improve public access to health care provider evaluation and investigation information.”

Is it licensed, or certified?
- Hospice provider, licensed by Minnesota’s Department of Health.
- Minnesota’s provider data gives it no Medicare number, so it isn’t Medicare-certified. CMS’s files list only the hospices Medicare certifies, so no CMS CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System., survey or inspection covers this one, and none is shown or compared.
- Licensee: nonprofit corporation.

Has the state acted against it?
- Minnesota’s provider data marks a conditional licence, and carries no other enforcement actions.
Who does the caring?
- Minnesota publishes no staffing levels in this list, and CMS’s staffing measures cover only certified agencies.
Who owns it?
- This list doesn’t name owners.
What do families say?
- CMS rates only Medicare-certified agencies, so no CMS rating is shown here; Minnesota publishes no rating in this list.
How does it compare nearby?
- Not compared: CMS’s measures cover only the hospices Medicare certifies, and this state’s list publishes no measure of its own to compare.
What the record doesn’t say
- Minnesota licenses every hospice provider, and its provider data lists each with any Medicare number. This page is one it gives none: not Medicare-certified, and so in none of CMS’s files. Branches, part of a licensee’s licence, and residential hospices aren’t listed on their own; a hospice CMS lists by the same name in the same city is left out, so none is listed twice.
- CMS’s files list only the hospices Medicare certifies, so no CMS star rating, survey or inspection covers this one, and none is shown or compared.
- Minnesota’s provider data carries no inspection results, so this page can’t say what inspectors found. The Department of Health says its Health Care Provider Evaluation and Investigation Results search “is designed to improve public access to health care provider evaluation and investigation information.”
- Minnesota’s provider data can list a licence past its expiration date while its renewal is processed.
