What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Plan of care (42 CFR 418.56(b))
The inspectors’ words, 239 words
“Based on record review and interview the agency failed to follow the individualized written plan of care established by the IDG in collaboration with the attending physician, the patient or representative, and the primary caregiver for 2 (Patient #5 and Patient #6) of 13 patient records reviewed. #5 Review of Patient #5s Plan of Care for certification period 01/07/2024 through 03/06/2024 revealed Patient #5 had declined Hospice Aide services as of 09/21/2023. Review of Patient #5s completed visits schedule revealed Patient #5 was receiving Hospice Aide services twice a week. Interview on 02/27/2024 at 3:40 p.m. with S1 DON revealed Patient #5s Hospice Aide services had resumed prior to the current certification period. S1 DON confirmed Patient #5s care plan had not been updated to reflect the resumption of aide services and should have been. #6 Review of Patient #6s 11/28/2023 – 02/25/2024 Plan of Care revealed a CNA visit frequency of 5 times a week starting 11/29/2023 and decreased to 3 times a week on 01/01/2024. Review of Patient #6s 11/28/2023 – 01/01/2024 CNA visit documentation revealed Patient #5 received 1 CNA visit the week of 11/28/2023 and 12/25/2023; 2 CNA visits the week of 11/03/2023; and 3 CNA visits the week of 11/10/2023 and 11/17/2023. Interview on 02/27/2024 at 1:15 p.m. with S1 DON confirmed the above findings. S1 DON stated Patient #6 should have been seen 5 times a week by a CNA and was not.”
