What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 419 words

“Based on record review, and interview, the hospice agency failed to ensure patient with wounds had documented weekly wound assessments for 1 (#1) of 3 (#1-#3) 3 patients reviewed for wound care. Findings: Patient #1 Review of the agency’s policy with a revision date of 04/09/2024 titled, “Wound Care”, Section titled “;Assessment and evaluation”, revealed in part: · “Documentation: Detailed documentation of wound assessments, treatments, and patient responses should be maintained in the medical record” Record review revealed patient#1 was admitted to the provider on 03/19/2024 with a primary diagnosis of malignant neoplasm of meninges along with diagnoses of aphasia following cerebral infarction, epilepsy, dependence on wheelchair, neoplasm related pain, nausea, shortness of breath, and hemiplegia affecting dominant side. Review of the initial skin assessment revealed there were multiple cancerous nodules to the scalp. The most recent certification period was from 06/13/2024 to 09/10/2024. The plan of care for the certification period called for the following home visits: Skilled Nurse- 2 times weekly with 10 as needed (prn) visits Hospice Aide- 3 times weekly Record review revealed skilled nurse visits had been made twice weekly as directed by the plan of care (POC). S2RegisteredNurse (RN) provided the skilled nurse visits documentation from 06/13/2024-07/30/2024. Each skilled nurse visit showed a record of wound care being provided as directed by the POC with a brief description of the wound in the narrative section of the notes. The narrative notes revealed S2RN recorded the nodules had an odor with oozing but did not have any details of wound care provided. Review of the “;skin assessment” section of the nurses visit note had no record of the wound or nodules to the scalp of patient#1. On 09/03/2024 at 1:25 p.m. an interview with S2RN confirmed she performed skilled nurse visits for the month of July 2024. S2RN reported the nodules were all over the scalp of patient #1 and along the front of her forehead. She described them as being raised, with a foul odor and oozing blood at times. S2RN could not confirm she recorded a detailed wound assessment on each skilled nurse visit. On 09/04/2024 at 08:25a.m., an interview with S1DON confirmed detailed weekly skin assessments were not documented in the skin assessment section for patient #1. S1DON confirmed the documentation in the narrative section of the skilled nurse`s notes was not sufficient. S1DON reported the wound assessment should have been recorded in the skin assessment section of the skilled nurse`s notes for each visit the skilled nurse completed for patient#1.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
