What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“Based on record review and interview, the agency failed to ensure the patient’s written individualized POC (plan of care) was followed as evidenced by the agency failing to ensure HA (hospice aide) visits were made as ordered per the POC (plan of care) for 2 (#7, #16) out of 19 sampled patients. 1. Review of the patient’s POC (Plan of Care) for benefit period 10/16/2023-01/13/2024 revealed an order for HA (Hospice Aide) visits 2wk13 (two times a week for 13 weeks). Review of the patients electronic clinical record for the week of 10/15/2023 revealed that there was no evidence 2 HA visits were made for that week as ordered per the benefit period plan of care. On 03/13/2024 at 3:25 p.m., a review of the patients electronic clinical record was conducted with S1Alt Adm (Alternate Administrator) and she confirmed that there was no evidence the 2 HA visits were made for the week of 10/15/2023.”
