What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Coordination of services (42 CFR 418.56(e)(2))

    The inspectors’ words, 411 words

“Based on document review and interview, it was determined that for 2 of 19 (Pt #13 and Pt #16) patients, the facility failed to ensure that care coordination for social work was completed per patient Plan of Care. Findings include:1. A review of the record for Pt #13 indicated the Plan of Care for the initial certification period of 4/24/2025 to 6/22/2025, documented the need for social work to see the patient once per month, for three months, with 2 additional PRN (as needed) visits. The record indicated an initial visit and assessment by the social worker was completed on 4/28/2025. No additional documentation was present in the record indicating the additional required visits were completed. There were no physician orders present indicating a change in the order for social work during this certification period.2. A review of the record for Pt #16 indicated the Plan of Care for the initial certification period of 7/18/2025 to 10/15/2025, documented the need for social work to see the patient once per month, for three months, with 2 additional PRN (as needed) visits. The record indicated an initial visit and assessment by the social worker was completed on 7/21/2025. No additional documentation was present in the record indicating the additional required visits were completed. There were no physician orders present indicating a change in the order for social work during this certification period. 3. The Policy titled “Hospice Plan of Care”, effective 9/17/2025, states “All hospice services furnished to patient and their families must follow the written Plan of Care.” 4. An interview was conducted on 9/18/2025 at approximately 11:50 AM, with the Director of Home Health and Hospice, (E#5). During discussion regarding the absence of documentation of the ordered social work visits, E #5 agreed the documentation was not there and noted this was due to the visits not being completed. E #5 stated she is “currently short one social worker” and has been working diligently to hire another, to ensure the patients are receiving their needed and ordered services. E #5 stated the current social worker is working “extremely hard to try to keep up, and works long hours” on a daily basis, but also noted she is having trouble doing so. E #5 stated she has been “working with the State to get an exception to the one-year rule” regarding social work experience, and has a “new grad (graduate)” candidate that is currently awaiting licensure, that she is planning to hire.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
