The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 5 deficiencies in 1 month of the last 36. Last cited Jan 2024. Most serious: Standard-level. Every month

What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“It is required that hospice volunteers provide day-to-day administrative or direct patient care services in an amount that, at a minimum, equals five percent (5%) of the total patient care hours of all paid hospice employees and contract staff. This standard was not met as evidenced by document review and interview. The agency was unable to provide evidence of volunteer hours for day to day administrative and/or direct patient care services provided. Review of the agency’s volunteer program/binder revealed hospice volunteer hours had yet to be logged for the agency. During a joint interview on 01/30/2024, the Administrator and Volunteer Coordinator stated that the agency has not yet needed to utilize a volunteer. After review of the standard the hospice acknowledged that the hospice is required to utilize volunteers to provide services that equals at least five percent of the total patient care hours, since the end of the public health emergency waiver beginning 01/01/2024. During an interview on 01/31/2024, the Administrator, the Volunteer Coordinator, and the Executive Director /Director of Patient Care Services /Alternate Administrator stated that the volunteer had performed 1 hour of service for the organization, but the agency was unable to provide the percentage of volunteer hours of the total patient care hours for all paid hospice employees and contract staff.”

In 1 of 8 (12%) clinical records reviewed, the agency did not ensure a complete hospice election statement was included in the record. Clinical Record 6: The patient elected the hospice benefit on 01/17/2024. Clinical record review revealed the hospice election of benefit statement did not include the effective date the patient elected the hospice benefit; the date was left blank but signed by the patient and clinician on 01/17/2024. During joint record review and interview on 01/30/2024, the Executive Director /Director of Patient Care Services /Alternate Administrator agreed that the hospice election statement was missing the effective date as required.”

    Rule L0673. Standard-level.

    The inspectors’ words, 155 words

Clinical Record 6: The patient elected the hospice benefit on 01/17/2024. The patient elected to revoke hospice care on 01/20/2024 and was transferred to the hospital for aggressive treatment. The clinical record contained no documented evidence that a discharge summary was forwarded to the receiving hospital. What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
