What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Rights of the patient (42 CFR 418.52(c)(2))
“Observed in Record Review. During record review it was noted that the patient had not been involved in making decisions about care, treatment and services. For example in record RRDC#2 the patient was discharged for extended prognosis however the notice of non coverage had not been signed 2 days before discharge from the organization. This was confirmed and discussed with the CEO, Compliance Officer and nurse present.”
-
Plan of care (42 CFR 418.56(b ))
The inspectors’ words, 141 words
“Observed in Record Review. In 5 of 10 patient records reviewed, it was noted that the written plan of care was not based on the patient’s goals and time frames, settings and services required to meet the goals. For example in record RRNHV#5 the patient had 5 open wounds with orders for dressings, nurse visits for wound care and IDG the nurse section described the wounds and orders, however there was no goal documented for skin integrity or wounds. For example in records RRNHV#2 and RRHV#2 the POC included spiritual goals for the spiritual counselor Provide non-judgmental listening -Assist in life review -Provide prayer, meditation, scripture reading but the spiritual counselor only visited the patient on admission and at recertification and there were only as needed visit orders. This was confirmed and discussed with the Compliance Officer, DPCS, CEO and nurse.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
