What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Plan Of Care
The inspectors’ words, 357 words
“Plan of care CFR(s): 484.60(a)(1) Each patient must receive the home health services that are written in an individualized plan of care that identifies patient-specific measurable outcomes and goals, and which is established, periodically reviewed, and signed by a doctor of medicine, osteopathy, or podiatry acting within the scope of his or her state license, certification, or registration. If a physician or allowed practitioner refers a patient under a plan of care that cannot be completed until after an evaluation visit, the physician or allowed practitioner is consulted to approve additions or modifications to the original plan. This Standard is not met as evidenced by: Based on interview and record review, it was determined the home health agency did not ensure care and services were provided as identified in the plan of care (POC) for 1 of 17 sampled patients. Specifically, aide visits were not provided as ordered in the POC. (Patient identifier: 14) Findings include: Patient 14 was admitted to the home health agency on 2/9/2024, with a diagnosis of atherosclerotic heart disease of native coronary artery without angina pectoris. Patient 14’s medical record was reviewed on 4/11/2024. Review of the plan of care (POC), developed for patient 14 on 2/9/2024, indicated that a home health aide was to visit the patient 1 time per week for 8 weeks. No documented evidence was found in the medical record to indicate a home health aide visit was completed for patient 14 from 2/9/2024 to 2/20/2024. In an email communication at 9:09 AM on 4/11/2024, surveyors asked if there was documentation regarding missed aide visits for patient 14 in the medical record. An email response from the agency was received at 9:28 AM on 4/11/2024, stating no aide visits had occurred for patient 14 between 2/9/2024 and 2/20/2024, due to the agency not receiving (health insurance company) authorization until 2/20/2024. In an interview with the director of nursing (DON) at 1:30 PM on 4/11/2024, the DON stated that although the insurance company had not yet provided authorization, aide visits should have been made because home health aide visits were on the plan of care orders for the patient.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
- Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
