The short answer

Record last released by CMS on Oct 21, 2026.

Each figure’s source sits beside it, in its section below.

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Plan Of Care

    The inspectors’ words, 458 words

“Plan of care CFR(s): 484.60(a)(1) Each patient must receive the home health services that are written in an individualized plan of care that identifies patient-specific measurable outcomes and goals, and which is established, periodically reviewed, and signed by a doctor of medicine, osteopathy, or podiatry acting within the scope of his or her state license, certification, or registration. If a physician or allowed practitioner refers a patient under a plan of care that cannot be completed until after an evaluation visit, the physician or allowed practitioner is consulted to approve additions or modifications to the original plan. This Standard is not met as evidenced by: Based on record review and interview, the facility failed to ensure services were provided as established in the written care of plan for one of 10 (Patient (P)9) sampled patients. A visit was not conducted per the physician signed plan of care and the physician was not notified in a timely manner. This deficient practice had the potential to cause the patient’s needs to not be met. Findings include: Review of the undated facility policy titled, “Clinical records” revealed, “Purpose: Establish guidelines for maintaining the individual record for every patient in accordance with professional standards . . . Missed Visit Coordination note is the documentation of services by the nurse, therapist. . . which includes. . . Notification to the agency and physician of the reason for the missed visit. . . “ Review of P9’s “Home Health Certification and Plan of Care,” located under the “Orders” tab in the electronic medical record (EMR), revealed a start of care (SOC) date of 02/15/25 with diagnoses of acute pain due to trauma, pain in left arm, Further review of the undated “Home Health Certification and Plan of Care” revealed Physical Therapist (PT) visits were to occur once a week for nine weeks, The plan of care was signed by the physician on 04/21/25. Review of the calendar, located under the “Medical Record” tab in the EMR, revealed missed visits on 04/16/25, 04/18/25 by the PT assistant and on 05/07/25 by the PT. The notes did not indicate notification to the physician for the missed visits. Review of the “Missed Visit Report” dated 05/14/25, provided by the facility indicated the physician was notified on 05/15/25 via fax. The notification went out 29 days after the first missed visit. During an interview on 05/21/25 at 8:15 AM, the Director of Operations (DOO) was asked about the missed visits and notification to the physician. The DOO stated that the patient cancelled the visits. The DOO also stated that the physician who are notified via fax should be every seven days, but the Administrative Assistant had been out, and they did not go out per the seven-day notification.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
  • Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
