What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Receive All Services In Plan Of Care
The inspectors’ words, 309 words
“Receive all services in plan of care CFR(s): 484.50(c)(5) Receive all services outlined in the plan of care. This Element is not met as evidenced by: Based on record review and interview, the facility failed to provide all services ordered on the plan of care for 1[P(patient) 4] of 13 (P1-P13) charts reviewed. This failure has the potential to negatively impact patient outcomes due to lack of care coordination and incomplete provision of ordered services. The findings are: A. Record review of the facility’s policy, Patient Rights/Notice of Privacy Practices, revised 04/2022, item 14, stated that the patient has the right to, “Receive all services outlined in their plan of care” B. Record review of P4’s medical record dated 03/22/26 revealed a provider-ordered plan of care that included PT (Physical Therapy), SN (Skilled nursing services) and MSW (Medical social work) services. C. Record review of a clinical note dated 04/02/26 documented by S(staff)7, clinical, revealed that the MSW attempted to contact P4 three times without success. S7 documented that the provider was notified by telephone, and the provider requested a faxed notification. The MSW then faxed a notification to the provider stating that the MSW visit would be rescheduled. D. Record review of the Episode Summary Report dated 04/16/2026 revealed that P4 was discharged from services. The report included “Medical Social Work for evaluation” as an ordered service; however, there was no documented evidence that the MSW evaluation was completed prior to discharge. E. During an interview conducted on 04/28/26 at 4:00 PM, S1, non-clinical, confirmed that the electronic medical record contained a fax to the physician dated 04/05/26 indicating that the MSW evaluation would be rescheduled. S1 verified that there was no documentation demonstrating the evaluation had been completed. S1 stated that P4 was discharged from the agency on 04/16/26 because the patient no longer met homebound criteria.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
- Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
