What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

A Review Of All Current Medications
The inspectors’ words, 241 words
“A review of all current medications CFR(s): 484.55(c)(5) A review of all medications the patient is currently using in order to identify any potential adverse effects and drug reactions, including ineffective drug therapy, significant side effects, significant drug interactions, duplicate drug therapy, and noncompliance with drug therapy. This Element is not met as evidenced by: Based on clinical record review and staff interview, it was determined the Agency failed to ensure a process was in place for checking all medications for actual or potential drug interactions in 1 (#5) of 8 (#1-#8) clinical records reviewed. This failed practice by the Agency presented a risk to all patients on census as the failure to conduct a complete medication review potentially delayed decisions regarding the patient’s medication regimen. Findings follow: Review of clinical record #5 revealed a patient with a start of care of 03/14/24 and a diagnosis of Chronic Kidney Disease Stage 4. A document titled “Home Health Care Certification and Plan of Care” with a certification period of 03/09/2025-05/07/2025 revealed orders for “Medical marijuana 10 mg dose; Inhale 3 puff(s) orally every 2 to 4 hours as needed for anxiety; Depression, pain Start Date: 03/14/2024” No documentation in the clinical record to indicate medical marijuana had been checked for potential side effects or interactions. An interview on 04/10/24 at 2:30 PM with Agency Personnel #4 confirmed the failure to check medical marijuana for actual or potential drug side effects or interactions.”

How do patients fare?
- CMS gives no CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.: “The number of patient episodes for this measure is too small to report.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
- Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
