What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 296 words
“IC-IF TO STATION=DISP/DEDICATE OR DISINFECT CFR(s): 494.30(a)(1)(i) Items taken into the dialysis station should either be disposed of, dedicated for use only on a single patient, or cleaned and disinfected before being taken to a common clean area or used on another patient. -- Nondisposable items that cannot be cleaned and disinfected (e.g., adhesive tape, cloth covered blood pressure cuffs) should be dedicated for use only on a single patient. -- Unused medications (including multiple dose vials containing diluents) or supplies (syringes, alcohol swabs, etc.) taken to the patient’s station should be used only for that patient and should not be returned to a common clean area or used on other patients. This Standard is not met as evidenced by: 22676 Based on observation, interview and policy review, the facility failed to ensure the hemostats and clamps used in patient care were disinfected appropriately. These findings have the potential to cause harm to all 49 patients receiving care at the facility. Findings include: Observation on the treatment floor on 12/17/24 at 11:45 AM, the hemostats used in patient care were observed to be soaking in the disinfectant in the closed position verses the open position two ensure adequate disinfectant of the interior surface of the clamp. In an interview with Manager of Clinical Services 4 on12/17/24 at 11:45 AM, confirmed the clamps were to be in the open positions when places in the disinfectant to ensure the inside of the clamps were adequately disinfected. Review of the facility’s policy titled, “Infection Control for Dialysis Facilities,” indicated, “Non-disposable items are to be disinfected ...Hemostats (blood line clamps) should be in the open position and fully submerged in an appropriate disinfectant solution ...Cleans areas should clearly be separated from contaminated areas where used supplies and equipment are handled.””
