What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Fac Receive/Review Self Monitoring Data Every 2 Months

    The inspectors’ words, 305 words

“H-FAC RECEIVE/REVIEW PT RECORDS Q 2 MONTHS CFR(s): 494.100(b)(2),(3) The dialysis facility must - (2) Retrieve and review complete self-monitoring data and other information from self-care patients or their designated caregiver(s) at least every 2 months; and (3) Maintain this information in the patient ’ s medical record. This Standard is not met as evidenced by: 40084 Based on interview and document review, it was determined the facility staff failed to review and address the lack of required data, namely blood pressure and weight information, on peritoneal dialysis (PD) flow sheets for one (1) of two (2) patients sampled (Patient #1). The findings include: On 01/16/2024 through 01/17/2024, the surveyor conducted a review of the medical record for Patient #1. That review included two (2) months of treatment flowsheets for the PD patient. A review of those flow sheets revealed the patient failed to record blood pressure and weight data for every treatment on the flow sheet for those two months. On 01/16/2024, the surveyor conducted an interview with Staff Member #3. Staff Member #3 advised they have been at the facility since August 2023. Since that time, Patient #1 has not documented blood pressure and weight data during home PD treatments. On 01/17/2024, the surveyor conducted an interview with Staff Member #1. Staff Member #1 advised they could not find documentation in the medical record related to the lack of required data reporting by Patient #1. A review of the facility policy titled “Review and Documentation of Home Therapies Patient Health Data” states in part: “At a minimum, the home therapies nurse will perform a comprehensive monthly review of all treatment data, regardless of the method of data submission. Data review will be discussed with the patient and/or care partner during the patient’s regular clinic visit and concerns will be addressed and documented accordingly.”
