What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Manage Volume Status
The inspectors’ words, 343 words
“POC-MANAGE VOLUME STATUS CFR(s): 494.90(a)(1) The plan of care must address, but not be limited to, the following: (1) Dose of dialysis. The interdisciplinary team must provide the necessary care and services to manage the patient’s volume status; This Standard is not met as evidenced by: 40084 Based on interview and record review, it was determined the facility failed to document in the patient’s treatment flowsheet that the patient’s weight could not be obtained prior to and after dialysis treatment due to a broken scale, and the information provided was based on an estimate (three of four flowsheets reviewed for Patient #1). The findings include: During an interview with Staff Member #1 on May 2, 2024 at 10:55 a.m., it was revealed the facility’s main scale to weigh patients prior to and after dialysis treatment was taken out of service on March 27, 2024. The facility arranged for a temporary scale to weigh patients but a small group of patients at the facility could not use the temporary scale due to physical limitations. The facility established a process for estimating the weight and the proper fluid to pull from these unique patients during dialysis. This included Patient #1. During a review of flowsheets for Patient #1 on May 2, 2024, the surveyor could not find any notes or other documentation indicating the patient could not be weighed prior to or after treatment, or that the fluid pulled during dialysis treatment was the results of an estimate. Staff Member #1 reviewed the flowsheets as well and could not locate information indicating that the treatment was any different than a typical treatment with Patient #1’s weight being obtained prior to and after treatment. A review of the facility’s policy titled “Patient Assessment and Monitoring” states in part: “Pre-Treatment... Follow the steps below for obtaining pre-treatment assessment data:... Weight... Record pre-weight. Compare pre-weight to estimated dry weight...” “Post-Treatment... Follow the steps below for obtaining post-treatment assessment data:... Weight... Obtain the patient’s post weight. Ensure the post weight is consistent with the goal set of the machine...””
