Each figure’s source sits beside it, in its section below.

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Pe - Hemodialysis Patients In View During Treatments

    The inspectors’ words, 330 words

“PE-HD PTS IN VIEW DURING TREATMENTS CFR(s): 494.60(c)(4) Patients must be in view of staff during hemodialysis treatment to ensure patient safety, (video surveillance will not meet this requirement). This Standard is not met as evidenced by: 30124 Based on observation, interview, and record review the facility staff failed to monitor patients’ vascular accesses to ensure patient safety by failing to ensure dialysis access were visible throughout the dialysis treatment for 1 of 10 stations (stations #9), during patients’ dialysis treatment. Allowing patients to cover access sites and line connections with blankets provides an opportunity for accidental needle dislodgement or disconnection to go undetected. This practice placed patients of the facility at risk for potential needle dislodgement and/or disconnection which could result in exsanguination and harm. Policy Reviewed of the facility’s current policy Title: Pre-Intra-post Treatment Data Collection, Monitoring and Nursing assessment; Policy: 1-01-08; Revision date April 2024. Purpose: To obtain and document baseline and ongoing information about the patient before, during and after the dialysis treatment through data collection and nursing assessment. This information will be used in planning and documenting the patient’s dialysis treatment, monitoring during treatment and for reviewing the patient’s response to the treatment and status prior to discharge. Intradialytic Date Collection/Assessment: 9. C. The vascular access site, blood line connections and the patient’s face should be visible throughout the dialysis treatment. Findings included: During a tour of the facility on 7/02/2024 along with staff #B the following was observed: Patient ID #5 at station #9 was observed to be receiving his dialysis treatment. Patient was observed receiving dialysis treatment with a blood flow of 350 ml/minute through a right chest central venous catheter. Patient #5 had her vascular access covered with blankets and not visible during their dialysis treatment. This finding was confirmed in an interview with the staff member #B on 07/02/2024. Staff was observed immediately uncovering the patient’s access and educating the patient to leaving their dialysis access uncovered and visible during dialysis.”
