Each figure’s source sits beside it, in its section below.

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Ic - Sanitary Environment

    The inspectors’ words, 280 words

“IC-SANITARY ENVIRONMENT CFR(s): 494.30 The dialysis facility must provide and monitor a sanitary environment to minimize the transmission of infectious agents within and between the unit and any adjacent hospital or other public areas. This Standard is not met as evidenced by: 28421 Based on observation, it was determined that the facility failed to practice effective infection control. Findings were: The Centers for Disease Control and Prevention (MMWR, No. RR-5) stated that “the hemodialysis machine and its components also can be vehicles for patient-to-patient transmission of blood borne viruses and pathogenic bacteria (24, 192). The external surfaces of the machine are the most likely sources for contamination. These include not only frequently touched surfaces (e.g., the control panel), but also attached waste containers used during the priming of the dialyzers, blood tubing draped or clipped to waste containers, and items placed on tops of machines for convenience.” Facility policy entitled, “Infection Control and Precautions for all Patients” stated in part, “Clean areas will be clearly designated and separated from contaminated areas and unused supplies and equipment ...Medications will be clearly designated as a clean area and separated from contaminated areas for the preparation, handling and storage of medications.” Tour of the treatment area on 3/4/25 revealed the following: * Labeled syringes on top of dialysis machine at station # 9 * A roll of tape on top of the dialysis machine at station # 14 * Tour of the lab area revealed a hole in the ceiling tile that was not fitted tightly to the pipe running through the ceiling. This hole could allow access to pests into the facility. These findings were confirmed by the Charge Nurse on 3/4/25.”

    Rule V0111. Standard-level.

  • Pe - Patient Care Environment - Sufficient Space

    The inspectors’ words, 144 words

“PE-PT CARE ENVIRONMENT-SUFFICIENT SPACE CFR(s): 494.60(c)(1) The space for treating each patient must be sufficient to provide needed care and services, prevent cross-contamination, and to accommodate medical emergency equipment and staff. This Standard is not met as evidenced by: 28421 Based on observation and interview, it was determined that the facility did not provide sufficient space between dialysis stations to prevent cross-contamination, and to accommodate medical emergency equipment and staff. Tour of the patient treatment area on 3/4/25 revealed 18 patient stations. Each patient station housed a chair and dialysis machine with roughly 2.5 feet between each patient. In case of an emergency, it is questionable that a Medical Emergency team could respond safely between patients. In the case of blood splatter, patients sitting next to each other are in danger of cross contamination. These findings were acknowledged by the Facility Administrator on 3/4/25.”

    Rule V0404. Standard-level.
