What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Ic - Wear Gloves/Hand Hygiene
The inspectors’ words, 255 words
“IC-WEAR GLOVES/HAND HYGIENE CFR(s): 494.30(a)(1) Wear disposable gloves when caring for the patient or touching the patient’s equipment at the dialysis station. Staff must remove gloves and wash hands between each patient or station. This Standard is not met as evidenced by: 35244 Based on policy review, observation and interview, the facility failed to ensure proper hand hygiene and CVC disinfection practices to prevent the spread of potentially infectious bacteria were performed by 1 of 5 (RN #1) staff observed during 1 of 2 (2/12/2024) observation days on the dialysis treatment floor. The findings included: 1. Review of the facility’s “INFECTION CONTROL FOR DIALYSIS FACILITIES” policy (April 2023) revealed, “...All teammates...will perform hand hygiene...prior to gloving and immediately after removal of gloves...after patient and delivery system contact...before touching clean areas such as supplies, supply cart and chairside keyboard/mouse...” 2. Observations on the treatment floor on 2/12/2024 beginning at 11:30 AM revealed RN #1 was disinfecting the CVC hubs for Random Patient #1 at station #12. RN #1 touched her glasses, adjusted her face mask, then went back to disinfecting the CVC hubs without first changing gloves and performing hand hygiene. RN #1 completed disinfection of the CVC hubs, moved a garbage can, removed gloves and donned new gloves, without first performing hand hygiene. In an interview on 2/14/2024 at 11:00 AM, the FA verified hand hygiene should have been performed after RN #1 touched her glasses and face mask, then new gloves applied. The FA also verified hand hygiene should be performed after removing gloves.”

Ic - Items Taken To Station Disposed/Dedicated Or Disinfected
The inspectors’ words, 289 words
“IC-IF TO STATION=DISP/DEDICATE OR DISINFECT CFR(s): 494.30(a)(1)(i) Items taken into the dialysis station should either be disposed of, dedicated for use only on a single patient, or cleaned and disinfected before being taken to a common clean area or used on another patient. -- Nondisposable items that cannot be cleaned and disinfected (e.g., adhesive tape, cloth covered blood pressure cuffs) should be dedicated for use only on a single patient. -- Unused medications (including multiple dose vials containing diluents) or supplies (syringes, alcohol swabs, etc.) taken to the patient’s station should be used only for that patient and should not be returned to a common clean area or used on other patients. This Standard is not met as evidenced by: 35244 Based on policy review, observation and interview, the facility failed to ensure supplies taken to the dialysis station were properly disposed of after possible contamination during 1 of 2 (2/12/2024) observation days on the dialysis treatment floor. The findings included: 1. Review of the facility’s “INFECTION CONTROL FOR DIALYSIS FACILITIES” policy (April 2023) revealed, “...Cleaning and/or disinfection of equipment and work surfaces will be performed as soon as possible following exposure to blood or other potentially infectious materials...” 2. Observations on the treatment floor on 2/12/2024 beginning at 11:58 AM, revealed Patient #5 in dialysis chair with oxygen delivered via bi-nasal cannula. The oxygen tubing became disconnected from the oxygen concentrator and was laying on the floor. RN #1 picked up the tubing and reconnected the tubing to the oxygen concentrator. RN #1 failed to discard the contaminated tubing and replace with new tubing. In an interview on 2/14/2024 at 11:00 AM, the FA verified the oxygen tubing should have been disposed of after falling on the floor.”
