What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Ic - Wear Gloves/Hand Hygiene
The inspectors’ words, 311 words
“IC-WEAR GLOVES/HAND HYGIENE CFR(s): 494.30(a)(1) Wear disposable gloves when caring for the patient or touching the patient’s equipment at the dialysis station. Staff must remove gloves and wash hands between each patient or station. This Standard is not met as evidenced by: 41879 Based on observations, interview, and review of the policy “Hand Hygiene”, Patient Care Technician (PCT) #1, one of three PCTs observed for standard precautions for infection control, failed to perform hand hygiene after glove removal. The findings included: On 04/01/2024 at 3:08 PM, observation in the treatment area of the clinic revealed PCT #1 discontinuing a dialysis treatment for a patient at Station 9. PCT #1 placed pre-torn tape, about seven pieces layered in a single strip, on the dialysis machine just above the keyboard.. PCT #1 touched multiple surfaces at the station including the dialysis machine, and also touched the patient with gloved hands. PCT #1 then went to the patient’s left arm fistula needle access site; and began removing the tape and needle without changing gloves and performing hand hygiene prior. PCT #1 continued fistula care. PCT #1 removed strips of tape from the dialysis machine and secured gauze over the access site once the needle had been removed. The finding was verified with PCT #1 during an interview on 04/01/2024 at 3:30 PM. Review on 04/04/24 of the policy provided by the dialysis clinic entitled “Hand Hygiene”, reads, “Decontamination using alcohol-based hand rub or by washing hands with anti-microbial soap and water should be done; before and after direct contact with patients, entering and leaving the treatment areas, before performing any invasive procedure such as vascular access cannulation or administration of parenteral medications, immediately after removing gloves, after contact with body fluids or excretion, mucous membranes, non-intact skin, and wound dressings if hands are not visibly soiled, after contact with inanimate objects near the patient.””
