What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Pe - Safe, Functional, Comfortable Environment
The inspectors’ words, 143 words
“PE-SAFE/FUNCTIONAL/COMFORTABLE ENVIRONMENT CFR(s): 494.60 The dialysis facility must be designed, constructed, equipped, and maintained to provide dialysis patients, staff, and the public a safe, functional, and comfortable treatment environment. This Standard is not met as evidenced by: 37898 Based on observation and interview, it was determined the facility failed to maintain a safe treatment environment by failing to ensure that expired supplies were not available for use. Findings: Observation of treatment room #3 on 01/24/2024 at 1025 revealed the following supplies available for use: - 7 Masterguard Plus 16 gauge fistula needle sets that expired 10/31/2023. - A bottle of Alcavis 50 disinfectant that expired 12/13/2023. During interview with the FA on 01/24/2024 at 1543, the above finding was confirmed. The FA stated that treatment room #3 was the least frequently used and, therefore, the room where any expired supplies were usually found.”

Rights Posted With State/Nw Contact #S And Addresses
The inspectors’ words, 298 words
“PR-RIGHTS POSTED,STATE/NW ONTACT INFO CFR(s): 494.70(c) The dialysis facility must prominently display a copy of the patient’s rights in the facility, including the current State agency and ESRD network mailing addresses and telephone complaint numbers, where it can be easily seen and read by patients. This Standard is not met as evidenced by: 37898 Based on observation and interview, it was determined the facility failed to prominently display a notice that included the current State agency mailing address and telephone complaint number that could be easily seen and read by patients. Findings: Observation of the facility lobby on 01/24/2024 at 1000 revealed the “Options for filing a complaint” notice posted on a bulletin board did not include the current State agency mailing address and telephone complaint number. During interview with the FA on 01/24/2024 at 1543, the above finding was presented. The FA accompanied the surveyors to the lobby and confirmed the finding. Observation of the facility lobby on 01/25/2024 at 0930 revealed the “Options for filing a complaint” notice posted on the bulletin board had been replaced with a new notice that included State agency contact information. However, the notice was posted on a paper significantly smaller than other information posted on standard 8.5x11 inch papers that were posted on the same bulletin board. The information was difficult to read unless standing in close proximity, directly in front of the bulletin board. During interview with the FA on 01/25/2024 beginning at approximately 1310, the additional finding was presented. The FA acknowledged that the form was neither prominently displayed, nor easily seen and read, and stated “I found our [corporate use] form but I shrunk it to fit the space I had available on the bulletin board. I didn’t read the part of the regulation about prominently displayed.””
